| Summary, etc. | Background <br/>To improve the management of patients with Crohn’s disease (CD), objective <br/>measurements of disease activity are needed. Ileocolonoscopy (CS) is the current <br/>reference standard but has limitations that restrict its repeated use. Bowel ultrasound <br/>(BUS), as a non-invasive tool, is potentially useful for activity monitoring. Superb <br/>microvascular imaging (SMI) is an innovative Doppler technique for assessing <br/>vascularity. <br/>Aim of work <br/>To assess the accuracy of BUS in diagnosing CD disease activity and severity <br/>compared to CS as the reference standard, and to develop a BUS-based activity <br/>scoring system in CD. <br/>Methods: <br/>In this cross-sectional analytic study, 58 adult patients with established ileocolonic <br/>CD were evaluated using BUS and colonoscopy. Endoscopic activity was recorded <br/>using the simple endoscopic score for Crohn's disease (SES-CD). BUS parameters <br/>that were assessed and compared to endoscopic activity include bowel wall thickness <br/>(BWT), color wall flow (CWF), power Doppler, bowel wall stratification, and <br/>perilesional findings. In addition, we added SMI as a novel parameter for the <br/>assessment of intramural vascularity in the affected bowel segments. <br/>Results <br/>Among the studied patients, 40 patients were in endoscopic activity, while 18 were <br/>in remission. The BUS variables that were significantly associated with endoscopic <br/>activity were BWT, CWF, cSMI, wall layering pattern, and perilesional <br/>lymphadenopathy (P < 0.05). The optimal BWT associated with disease activity <br/>was ≥ 5mm with a sensitivity and specificity of 90.0%, 66.67, respectively (AUC: <br/>0.740). After applying further univariate then multivariate analyses on these <br/>parameters, only cSMI grade ≥ 2 was found to be the most significant parameter <br/>associated with endoscopic activity with OR (95% CI) of 5.187 (1.478–18.201) and <br/>with p-value = 0.001. <br/>Conclusion <br/>BUS is a non-invasive procedure for assessing disease activity and monitoring <br/>disease course in patients with CD, reducing the risks due to the invasiveness of <br/>colonoscopy together with reducing health care costs. In addition, SMI may offer <br/>enhanced diagnostic capabilities in evaluating intramural vascularity and <br/>subsequently activity in CD. |

| Summary, etc. | يعتبر تقييم درجة نشاط الالتهاب في المرضى الذين يعانون من مرض كرون أمرا ضروريا. ولتحقيق ذلك، هناك حاجة إلى قياسات موضوعية لتقييم نشاط المرض. منظار القولون يظل هو المعيار الأساسي لتقييم هذا النشاط، ولكن له قيود تحد من استخدامه المتكرر، ما يجعل المؤشرات الأخرى الغير تداخليه أكثر قبولا وأقل تكلفة بالنسبة للمرضى. تعتبر الموجات فوق الصوتية على الأمعاء أحد الأدوات الغير تداخلية المفيدة في متابعة مثل هذه الحالات، ويعد التصوير الفائق للأوعية الدموية الدقيقة (ٍSMI) أحد تقنيات الدوبلر الحديثة لتقييم الأوعية الدموية. <br/>هذه دراسة تحليلية مستعرضة شملت ٥٨ مريضًا بمرض كرون. تم تجميع المرضى من مركز دراسات و أبحاث أمراض الأمعاء بقسم الأمراض المتوطنة، كلية الطب، جامعة القاهرة.<br/>هدفت دراستنا إلى معرفة مدى دقة الموجات فوق الصوتية على الأمعاء في تحديد نشاط وشدة التهاب الأمعاء في المرضى الذين يعانون من مرض كرون، وتطوير معايير كمية قائمة على هذا الفحص، ومقارنة هذا التقييم بالتقييم المتبع القائم على منظار القولون. <br/> تمت هذه الدراسة عن طريق تجميع المرضى المسبق تشخيصهم بمرض كرون. تم عمل موجات فوق صوتية على الأمعاء ، والتي تمت مقارنتها بالمنظار. تم تسجيل نشاط المرض بالمنظار باستخدام نظام الترقيم التنظيري البسيط لمرض كرون. الدلالات التي تم تقييمها بالموجات فوق الصوتية شملت: سُمك جدار الأمعاء، الأوعية الدموية بجدار الأمعاء باستخدام التصوير بالأمواج فوق الصوتية الملون والدوبلر، بالإضافة إلى التصوير الفائق للأوعية الدموية الدقيقة (ٍSMI)، كما تم أيضا تقييم الأنسجة المحيطة بالأمعاء. <br/>تم تقييم ٥٨ مريضا في هذه الدراسة؛ كان ٤٠ مريضا منهم يعانون من مرض نشط، بينما كان ١٨مريضا في حالة هدأة مرضية (وفقا للمنظار). أظهر المرضى الذين يعانون من مرض نشط سُمكا أكثر في جدار الأمعاء وكذلك نسبة أعلى في الأوعية الدموية التي ظهرت في جدار الأمعاء باستخدام (CWF) و التصوير الفائق للأوعية الدموية الدقيقة (ٍSMI)، كما أنهم كانت لديهم غدد ليمفاوية محيطة بالأمعاء بشكل أكبر من أولئك الذين كانت لديهم هدأة مرضية . ويعتبر سُمك جدار الأمعاء ≥ ٥ مم هو القيمة الأمثل المرتبطة بنشاط المرض بنسبة حساسية تقدر بـ ٩٠ ٪، وخصوصية تقدر بـ ٦٦٬٦٧٪. تم إجراء تحليل أحادي المتغيرات، ثم متعدد المتغيرات على هذه الدلالات، والذي أظهر أن التصوير الفائق للأوعية الدموية الدقيقة بدرجة ≥ ۲هو العامل الأكثر ارتباطا بنشاط المرض الذي تم قياسه باستخدام نظام الترقيم التنظيري البسيط. <br/>أظهرت هذه الدراسة أهمية استخدام الموجات فوق الصوتية لتقييم نشاط وشدة الاتهابات في المرضى الذين يعانون من مرض كرون. كما أبرزت هذه الدراسة، أيضا، أهمية التصوير الفائق للأوعية الدموية في جدار الأمعاء لتقييم نشاط وشدة الالتهابات في المرضى الذين يعانون من مرض كرون |
