The main objectives of the study:<br/>To compare effectiveness of intra oral technique of using TAD-supported class III elastic wear versus extra oral face mask appliance for maxillary protraction and correction of reverse overjet following ALT-RAMEC protocol using hybrid maxillary rapid expander for treating growing class III patients with maxillary deficiency and compare patient acceptance using these appliances<br/>b. The methods used:<br/>24 class III growing (CVM 2-CVM 3) patients aged 9-13 years old with maxillary deficiency treated with hybrid maxillary rapid expander anchored on two palatal mini-screws with a posterior bite plane following an ALT-RAMEC protocol for nine weeks followed by protraction phase for 6 months using intra oral TAD-supported class III elastics extending from modified lower lingual arch in intervention group and extra oral facemask in control group. After finishing maxillary protraction, patients were asked to respond to patient satisfaction questionnaire regarding the appliances used to evaluate patients’ treatment experience and their satisfaction with the results and child perception questionnaire to evaluate oral health related quality of life.<br/>c. Summary of the results:<br/>All patients in intervention group were successfully treated reaching positive overjet and overbite while three patients in control group reached only edge to edge relationship because of patient incompliance.<br/>There was significant advancement of A point in both groups with mean change in group using intra oral appliance was slightly higher than group using extra oral appliance but difference was statically insignificant .Mandibular base redirected posteriorly more in intervention group than in control group but the difference was not statically significant.<br/>There were minimal maxillary dental changes in both groups with some retraction of upper incisors which was negligible. However, Mandibular incisors had retroclined slightly more in control group than in intervention group but difference was not statistically significant. Regarding vertical control, there was a significant increase in SN/MP in control group but wasn’t statically significant when compared to intervention group |

| Summary, etc. | لمقارنة فعالية تقنية داخل الفم باستخدام اساتك من الدرجة الثالثة مدعومة بالمسامير التقويمية مقابل جهازقناع الوجه<br/>الخارجي لإطالة الفك العلوي وتصحيح الدرجة القاطعة السهمية العكسية باتباع بروتوكول الالت راميك باستخدام جهاز -<br/>موسع الفك العلوي المهجن لعلاج مرضى الفئة الثالثة المتزايدة الذين يعانون من قصور في الفك العلوي ومقارنة قبول<br/>المريض باستخدام هذه الأجهزة<br/>ب الطرق المستخدمة : -<br/>عاما يعانون من نقص في نمو الفك ١٣-٩ مريضا من الفئة الثالثة في مرحلة النمو ) المرحلة الثانية و الثالثة( عمرهم ٢٤<br/>العلوي تم علاجهم بواسطة جهاز هجين موسع سريع للفك العلوي مثبتين على مسمارين صغيرين في سقف الحلق مع<br/>أشهر باستخدام أشرطة ٦ اسابيع يليها مرحلة للاطالة لمدة ٩ مستوى عضة خلفي مع اتنباع بروتوكول الت راميك لمدة -<br/>مطاطية داخل الفم من الفئة الثالثة مدعومة من الداعمات التقويمية تمتد من القوس اللغوي السفلي المعدل في مجموعة<br/>التدخل وقناع الوجه خارج الفم في المجموعة الضابطة. بعد الانتهاء من إطالة الفك العلوي ، طلب من المرضى الرد على<br/>استبيان رضا المرضى فيما يتعلق بالأجهزة المستخدمة لتقييم تجربة علاج المرضى ورضاهم عن النتائج واستبيان إدراك<br/>الطفل لتقييم جودة الحياة المتعلقة بصحة الفم.<br/>ج ملخص النتائج : -<br/>تم علاج جميع المرضى في مجموعة التدخل بنجاح للوصول إلى فرط إيجابي وفرط العض بينما وصل ثلاثة مرضى في<br/>المجموعة الضابطة إلى علاقة من الحافة إلى الحافة فقط بسبب عدم امتثال المريض. كان هناك تقدم شديد لنقطة )أ( في كلا<br/>المجموعتين مع متوسط التغير في المجموعة التي تستخدم الأجهزة داخل الفم كان أعلى قليلا من المجموعة التي تستخدم<br/>جهازا من خارج الفم ولكن الفرق كان ضئيلا احصائيا. تمت إعادة توجيه قاعدة الفك السفلي إلى الخلف في مجموعة التدخل<br/>أكثر من المجموعة الضابطة ولكن الفرق كان ضئيلا احصائيا. . كان هناك تغيرات ضئيلة في الأسنان في الفك العلوي في كلا المجموعتين مع بعض التراجع في القواطع العلوية و لكنه<br/>كان ضئيلا ولكن قواطع الفك السفلي قد تراجعت قليلا في مجموعة السيطرة اكثر من مجموعة التدخل ولكن الفرق لم يكن<br/>ذا دلالة إحصائية. فيما يتعلق بالتحكم الرأسي, كانت هناك زيادة كبيرة في زاوية الدالة علي التغييرات العظمية في الاتجاه<br/>الرأسي لكنها لم تكن ذات دلالة ثابتة عند مقارنتها بمجموعة التدخل. كان وقت العلاج أقصر بكثير في مجموعة التدخل<br/>ربما بسبب طبيعة الأجهزة في داخل الفم في مجموعة التدخل التي ساعدت في وضع الأشرطة المطاطية لفترة أطول في<br/>اليوم مقارنة بقناع الوجه الذي يكون خارج الفم في المجموعة الضابطة التي قوبلت بعدم قبول المريض.<br/>كان المرضى الذين يستخدمون الأجهزة داخل الفم أكثر رضا عن تجربة العلاج والنتائج النهائية من المرضى الذين<br/>يستخدمون أجهزة خارج الفم وفقا لمقياس الرضا العام الكلي الذي تم جمعه من استبيان رضا المرضى. |
