Summary, etc. | Background: Since its original description, the triple pelvic osteotomy has undergone several<br/>modifications and refinements. Triple pelvic osteotomy can be done in patients with open or<br/>closed triradiate cartilage. One major advantage of Ganz periacetabular osteotomy (PAO) is<br/>preserving the posterior column intact, preservation of the posterior column adds to the stability<br/>of the hemipelvis thus allowing early mobilization and weight bearing, preservation of the<br/>acetabular blood supply, maintenance of hip abductor musculature, in addition to sciatic nerve<br/>protection. The juxta-articular osteotomy planes offer the ability to effect better deformity<br/>correction about an ideal center of rotation. While maximizing joint stability, coverage and<br/>congruency, the acetabular reorientation must also be assessed in light of the impingement-free<br/>range of motion.<br/>The value of redirectional osteotomies that they reduce hip subluxation and medialize the<br/>hip joint center, redistribute the contact stresses from the acetabular rim only to the whole<br/>articular surface, in addition to transforming the shear stresses across the articular cartilage into<br/>compressive stresses, thus preserving the articular cartilage (12).<br/>The aim of our current study is to ass the functional and radiological outcome of triple pelvic<br/>osteotomy in management of late onset acetabular dysplasia.<br/>Methods: The study is a prospective case series study that recruited 22 hips in 18 patients,<br/>in the period from April 2021 to October 2023 with a minimum follow up of six months.<br/>Symptomatic patients with radiographic evidence of acetabular dysplasia, who had an adequate<br/>hip range of motion, with the age between 8 and 18 years, were included. Demographic data,<br/>underlying diagnosis, and preoperative radiographic data were recorded. Intraoperative details<br/>including estimated blood loss and type of postoperative immobilization were noted. Over the<br/>follow-up period, complications were recorded as were radiographic outcomes including lateral<br/>center edge angle, acetabular index, migration percentage, continuity of Shenton's line, and time<br/>to union.<br/>Results: The average age of the included patients in this study was 14.7 years, the average<br/>BMI of the included patients was 22.8 kg/m2. There were 14 girls (63.6%), and 8 boys (36.4<br/>%). The female: male ratio was nearly 3:2. Previous ipsilateral hip surgeries were reported in 8<br/>patients (36.3%) ; three cases underwent adductor tenotomy and closed reduction, and five cases<br/>had adductor tenotomy, open reduction and capsulorrhaphy. The functional results were<br/>evaluated using HHS. The overall final clinical results according to HHS score were excellent<br/>in three hips (13.6%), good in 15 (68.2%), fair in 2 hips (9.1%) poor in 2 hips (9.1%). The<br/>average increase of HHS after surgery was 2.0 ± 3.18 points. Complications were encountered<br/>in 6 cases (27.3%). Anterior column fracture occurred in one patient (4.5%). Lateral femoral<br/>cutaneous nerve dysesthesia was encountered in two patients (9%) early after surgery. One<br/>12<br/>The functional results were evaluated using HHS. Two patients (9%)<br/>suffered from sciatic nerve injury, diagnosed clinically and by nerve<br/>conduction test. One patient (4.5%) developed gradual sublaxation until<br/>finally dislocated after 5 months from surgery.<br/>Conclusions: In this single-center prospective series, short term<br/>outcome of triple pelvic osteotomy for correction of acetabular dysplasia<br/>in adolescents shows good functional and radiological outcomes, with low<br/>complications risk.<br/>Level of evidence: Level IV-case series. |

Summary, etc. | يعتبر تشوه عظمة الحق في المراهقين من الأمراض التي تؤثر على البناء التركيبي و الديناميكي لمفصل الحق بشكل كبير، ويحدث ذلك نتيجة وجود تسطح للمفصل مما يؤدي لعدم تغطية رأس عظمة الفخذ بالشكل الكافي<br/>وتتراوح نسبة هذا المرض الى بين 3 الى 5 في المئة من حديثي الولادة، وتكون النسبة الأكبر في النساء، ويحدث تحسن بشكل تلقائي في نسبة كبيرة من الحالات مع مرور الوقت، أما في حالة استمراره حتى سن المراهقة، فقد يؤدي إلى حدوث آلام مزمنة وقد يؤثر على حركة المفصل بشكل كبير مما يستدعي التدخل الجراحي.<br/>وينقسم العلاج في هذه الحالات الى علاج تحفظي او علاج جراحي، وعادة ما يكون التدخل الجراحي بإحداث قطع عظمي بعظمة الحق بطرق مختلفة.<br/>وتعتمد فكرة القطع العظمي على تغيير شكل أو اتجاه المفصل ليتم تغطية رأس عظمة الفخذ بشكل كاف وملائم للمحافظة على استقراره وتجنب حدوث المضاعفات. <br/>الهدف من الدراسة<br/>تهدف هذه الدراسة إلى تقييم نتائج القطع العظمي حول مفصل الحق لإصلاح تشوه مفصل الحق في سن الطفولة المتأخرة<br/>الحالات <br/>المرضى والوسائل: تم اجراء الجراحة على 22 مريض في الفترة بين ابريل 2021 وحتى اكتوبر 2023 مع تحديد فترة متابعة المريض بحد أدنى ستة أشهر.<br/>الحلات اللي تم الحاقها بالبحث: الحالات اللي تعاني من تشوه بمفصل الحق في المراهقين بين سنة الثامنة والواحد و عشرون عاما، مع وجود مدى حركي مقبول للمفصل.<br/>الحالات المستثناة من البحث: الحالات التي تعاني من تأثر كبير بالمدى الحركي للمفصل، أو تكون مفصل حق جديد أعلى المفصل الطبيعي، أو عدم وجود توافق بين حجم الرأس ومفصل الحق<br/>مواصفات المرضى <br/>• السن: كان متوسط اعمار المرضى 15 سنة وتتراوح أعمارهم بين 8 الى 21 سنة.<br/>• النوع : 14من الإناث و 8 من الذكور<br/>• الناحية التي تعانى من التشوه: المفصل الأيسر في 14 مريض والمفصل الأيمن في 8 مرضى <br/>• متوسط كتلة الجسم: كان متوسط كتلة الجسم في المرضى المشمولين بالبحث 22.8 كيلوجرام بالمتر المربع، مع وجود سبعة مرضى بوزن فوق المتوسط<br/>• تقسيم المرضى تبعا لتقسيم سيفرين: خمسة من المرضى كانوا من الدرجة الثانية، تسعة من الدرجة الثالثة، وثمانية من الدرجة الرابعة<br/>• مقياس زاوية الحق: كان متوسط مقياس زاوية الحق قبل اجراء الجراحة هو 9.02± 34.32 درجة<br/>•مقياس الزاوية بين مركز رأس عظمة الفخذ وحافة عظمة الحق الخارجية : كان متوسط مقياس هذه الزاوية قبل اجراء الجراحة هو 14.05 ± 4.31 درجة<br/> |
