| Summary, etc. | AO classification is the classification used in this study and it helps guide<br/>the plan of treatment and the method of fixation as well as the expected<br/>intraoperative difficulties and postoperative complication; where Q2,3,4 and 5 are<br/>fixed by miniplate 2.7 and Q 6 is fixed by small DCP 3.5 mm when 6 cortices<br/>distal to the fracture site is feasible. Q1 is out of the scope of this study.<br/>Unlike distal radius fracture, distal ulna fracture isn’t widely studied and<br/>there are still areas of research to identify the best treatment.<br/>This study aims to assess the outcome of volar plating of distal ulnar<br/>fractures using mini plates 2.7 mm in Q2,3,4 and 5 and small DCP in Q6 fractures.<br/>The approach used in this study is the classic approach between FCU and<br/>ECU and provides a safe and sound exposure to the volar aspect of the- distal ulna<br/>with the ability to safely identify and protect the dorsal branch of ulnar nerve. It’s<br/>volar plating not a volar approach. The adequate soft tissue cuff and putting the<br/>plate away from ECU saves the pt from ECU tendinitis and allows painless wide<br/>ROM.<br/>This prospective study was conducted on 27 patients with a mean age of 40.6<br/>years (18-65) with distal ulna fx, all of them were managed by open reduction and<br/>internal fixation, 14 cases with small DCP, and 13 cases with mini plate 2.7 mm<br/>using volar plate technique |

| Summary, etc. | تشير الدراسة: إلى أن نسبة كسور الزند البعيدة كانت 74 لكل 100,000 شخص سنوياً. وأكثر من 40% من الكسور تحدث مع إصابة مصاحبة للكعبرة البعيدة. تم استخدام تصنيف AO في هذه الدراسة وكان مفيداً في توجيه خطة العلاج وطريقة التثبيت وكذلك التوقعات بالصعوبات أثناء العملية والمضاعفات بعد العملية؛ حيث تم تثبيت الكسور من النوع Q2,3,4 و5 بواسطة شريحة صغيرة 2.7mini والنوع Q6 بواسطة DCP إذا كان ذلك ممكناً بستة قشور عظمية بعيدة عن موقع الكسر. النوع Q1 لم يتم تغطيته في هذه الدراسة. على عكس كسور اسفل عظمة الكعبرة، لم يتم دراسة كسور الزند البعيدة بشكل واسع ولا تزال هناك مجالات للبحث لتحديد أفضل علاج. الهدف من هذه الدراسة هو تقييم نتائج تثبيت الزند البعيدة بواسطة شرائح mini 2.7 مم في الكسور من النوع Q2,3,4 و5 وشريحة صغيرة ضاغطة في الكسور من النوع Q6. الطريقة المستخدمة في هذه الدراسة هي الطريقة الكلاسيكية بين عضلتي FCU و ECU وتوفر تعرضاً آمناً وواضحاً للجانب البطني للزند البعيد مع القدرة على تحديد وحماية فرع العصب الزندي الظهري بأمان. تم إجراء هذه الدراسة على 27 مريضاً بمتوسط عمر 40.6 سنة (18-65) مع كسور الزند البعيدة، وتم علاجها بواسطة رد مفتوح وتثبيت داخلي، 14 حالة بواسطة شريحة صغيرة ضاغطة 3.5مل و13 حالة بواسطة شريحةmini 2.7 مم باستخدام تقنية الشريحة الامامية. كانت أقصر فترة متابعة 6 أشهر. تم إجراء جميع الحالات في الأسبوع الأول بعد الإصابة، 50% منها في اليومين الأولين. أظهرت الدراسة أن متوسط الوقت للمريض لتحقيق الالتحام كان 8±2أسبوع. هناك أهمية إحصائية بين نطاق الحركة وتصنيف الكسر، حيث كان لدى Q6 مزيد من الانثناء للأمام والخلف مقارنة بكسور Q2,3,4 و5. التقييم بعد 6 أشهر من العملية: كان متوسط درجة الألم حسب مقياس VAS هو .74±.76، وقوة القبضة للجانب السائد كانت: 47.2±8.2 كغم بينما للجانب غير السائد كانت: 44.2±8.5 كغم وكان معدل اعاقات الذراع والكتف واليد DASH 11.89±11.57. 24<br/>بالنسبى لمجال الحركة كان معدل انثناء مفصل الرسغ بعد ستة اشهر من المتابعة 52.41±4.01 ومعدل الفرد 58.00±10.48 ومعدل الدوران للخارج 82.85±3.84 ومعدل الدوران للداخل 81.48±3.04 <br/>88% من الحالات لم تكن لديها مضاعفات. بينماحدث خشونة المفصل بين اسفل عظمتي الكعبرة والزند لحالة، بعد 6 أشهر من المتابعة، وتحسنت سريرياً بشكل ملحوظ على العلاج الطبيعي والمسكنات. حالة واحدة فشلت و خضع المريض لمراجعة العملية بإزالة الشريحة رد مفتوح وتثبيت داخلي بواسطة شريحة mini 2.7 مم وزرع عظم ولا يزال تحت المتابعة بعد العملية الثانية. وحالة واحدة كانت لديها إصابة مؤقتة في فرع العصب الزندي الظهري مع تنميل، تحسن المريض على حقن الكورتيكوستيرويد الموضعي. لم يعاني أي من المرضى من التهاب وتر العضلة الباسطة الزندية أو العدوى.<br/> |
