| Summary, etc. | Objective: The present randomized controlled trial evaluated for the first time the application of autogenous demineralized dentin graft with injectable platelet rich fibrin (ADDG+i-PRF; test-group) versus autogenous demineralized dentin graft (ADDG; control-group) alone, in alveolar ridge preservation (ARP) of post extraction sockets in the maxillary aesthetic zone. <br/>Materials and methods: Twenty-two maxillary (n=22) non-molar teeth indicated for extraction were randomized into two groups (n=11/group). Extracted teeth were prepared into ADDG, implanted into extraction sockets with or without i-PRF amalgamation, and covered by collagen sponge. Cone-beam computed tomography (CBCT) scans at baseline and six months were compared to assess ridge-dimensional changes. Additionally, width of keratinized tissue, postoperative patient satisfaction, pain score and chair time were recorded. In the course of subsequent dental implant placements at six months, bone core biopsies of the engrafted sites were obtained and analyzed histomorphometrically. |

| Summary, etc. | الحفاظ على الحافة السنخية هو إجراء ينطوي على وضع مادة التطعيم في تجويف السن المقلوع للحد من التغيرات في الأبعاد بعد القلع. تم اقتراح مواد ترقيع مختلفة في الأدبيات لملء تجويف السنخ المقلوع، من بينها طعم العاج. تتمثل المزايا الرئيسية لتطعيم عاج الأسنان في أنه يشترك في التركيب الكيميائي الحيوي للعظم السنخي، وكونه ذاتي المنشأ، وفعال من حيث التكلفة، وخالٍ من التفاعلات المستضدية ويحتوي على العديد من عوامل النمو التي تعزز من إمكانات التطعيم في تكوين وإنتاج العظام. <br/>مركزات الصفائح الدموية هي مصدر آخر لعوامل النمو التي يمكن استخدامها في الحفاظ على الحافة السنخية. والفيبرين الغني بالصفائح الدموية القابل للحقن هو نوع من مركزات الصفائح الدموية التي تعتمد على مفهوم الطرد المركزي منخفض السرعة وتميل إلى إصدار تركيز أعلى من عوامل النمو ووقت إطلاق ممتد. ويمكن استخدامه مع الطعوم العظمية في عمليات مختلفة مما يوفر ثباتًا وسهولة في التعامل مع الطعم العظمي المستقر الناتج عن الفيبرين"الطعم اللزج".<br/>كان الهدف من هذه التجربة السريرية العشوائية هو تقييم عرض الحافة الشدقية اللسانية سريريًا وشعاعيًا (النتيجة الأولية)، وكذلك ارتفاع الحواف الشدقية واللغوية للسنخ ودرجة الألم ورضا المريض ووقت الكرسي وعرض الأنسجة المتقرنة (النتائج الثانوية) بعد استخدام طعم الفيبرين الغني بالصفائح الدموية القابل للحقن وطعم العاج الذاتي منزوع المعادن مقارنةً بطعم العاج الذاتي منزوع المعادن وحده. بالإضافة إلى ذلك، هدفت الدراسة إلى تقييم جودة العظام من الناحية النسيجية.<br/>تم تضمين عشرين من المشاركين الأصحاء مع اثنين وعشرين من أسنان الفك العلوي غير القابلة للترميم الممتدة من الضاحك الثاني إلى الضاحك الثاني وتم اختيارهم بصورة عشوائية في مجموعتين. بدأ الإجراء الجراحي بعملية القلع الغير مؤذي. تم تنظيف السن المخلوع جيدًا وتم إجراء تحضير طعم العاج باستخدام مقراض العظام ومطحنة العظام. وأعقب ذلك نزع المعادن، والتنظيف مرتين بمحلول ملحي وتجفيفه بشاش معقم. تم تحضير الفيبرين الغني بالصفائح الدموية عن طريق الحقن بعد جمع 10 ملم من الدم الوريدي في أنبوب بلاستيكي والطرد المركزي عند 700 دورة في الدقيقة لمدة 3 دقائق. تلقت المجموعة الضابطة طعم عاج منزوع المعادن ذاتي المنشأ وحده بينما تلقت مجموعة التدخل طعم عاج منزوع المعادن ذاتي المنشأ + الفيبرين الغني بالصفائح الدموية القابل للحقن، وتم تغطية كلاهما بإسفنجة الكولاجين وتم تأمينهما بعقدة متقاطعة باستخدام مادة خياطة البولي بروبيلين 5-0. تم قياس عرض الأنسجة المتقرنة بعد القلع وبعد 6 أشهر بعد العملية. بالإضافة إلى ذلك، تم الحصول على رضا المريض بعد العملية الجراحية ووقت الجلوس ودرجة الألم بعد الانتهاء من العلاج. تم الحصول على خزعات عظمية أثناء وضع الزرعة بعد 6 أشهر لتقييم التئام العظام نسيجيًا. |
