| Summary, etc. | Aim: The aim of the present study was to compare the effect of using diclofenac potassium versus prednisolone, as a pre-medication, compared to placebo on intensity and incidence of post-endodontic pain, and pulpal IL-8 expression in patients with symptomatic irreversible pulpitis. <br/>Methods: Thirty-six patients having mandibular premolars diagnosed with symptomatic irreversible pulpitis were included in this study. After confirming the diagnosis clinically and radiographically, patients were randomly assigned into one of three groups (n=12). Intervention groups included single oral dose of 50 mg diclofenac potassium pre-medication one hour before starting endodontic treatment, or single oral dose of 30 mg prednisolone pre-medication one hour before starting endodontic treatment. Placebo was used in the control group one hour before starting endodontic treatment. Standard endodontic treatment was performed in a single visit using ProTaper Next files for instrumentation and cold lateral compaction technique with resin-based sealer for obturation. Post-endodontic pain intensity was assessed immediately and at 6, 12, 24 and 48 hours after treatment in addition to the incidence of post-endodontic pain. Visual analogue Scale (VAS) was used as the primary outcome measure. Secondary outcome comprised the quantification of pulpal IL-8 expression (ng/L) using ELISA. <br/> Results: There was no significant difference between the three groups in mean age, gender and teeth distribution. With regards to immediate post-endodontic pain intensity, there was a statistically significant difference between pain scores in the three groups (P= 0.010) where Placebo group showed a statistically significant highest pain score and no difference between Prednisolone and Diclofenac Potassium groups. In addition, there was no statistically significant difference between pain scores in the three groups at 6 (P= 0.479) and 12 hours (P= 0.085). At 24 and 48 hours, none of the patients in the three groups reported post-endodontic pain. Furthermore, there was no statistically significant difference in the incidence of post-endodontic pain (P= 1) and in mean IL-8 levels between the three groups (P= 0.193).<br/>Conclusions: Within the limitations of the present clinical trial, it was concluded that prednisolone and diclofenac potassium groups offered equal control of immediate post-endodontic pain, where placebo group had the highest immediate post-endodontic pain score. Similarly, prednisolone and diclofenac potassium groups equally influenced post-endodontic pain incidence as well as pulpal interleukin-8 expression. |

| Summary, etc. | شملت هذه الدراسة ستة وثلاثين مريضاً لديهم ضواحك الفك السفلي أحادية القناة وتم تشخيصهم بالتهاب اللب غير الردود العرضي. بعد تأكيد التشخيص سريريا وشعاعيا، تم توزيع المرضى بشكل عشوائي إلى واحدة من ثلاث مجموعات (ن = ١٢). شملت مجموعات التدخل جرعة واحدة عن طريق الفم من ٥٠ ملغ من ديكلوفيناك البوتاسيوم قبل ساعة واحدة من علاج الجذور، أو جرعة واحدة عن طريق الفم من ٣٠ ملغ من بريدنيزولون قبل العلاج بـ ٣٠ دقيقة قبل بدء علاج الجذور. تم استخدام الدواء الوهمي في المجموعة الضابطة قبل ساعة واحدة من بدء علاج الجذور.<br/>تم إجراء المعالجة اللبية في زيارة واحدة وتم تحديد طول قنوات العصب باستخدام جهاز محدد الاطوال الإلكتروني والأشعة الداخل فمويه، ثم تحضير القنوات باستخدام الملفات الدوارة بروتابر نكست، ثم حشو القنوات من خلال تقنيه الكبس الجانبي باستخدام جاتبركا ورزن سيلر. تم تقييم الألم بعد العلاج على الفور وبعد ٦ و١٢ و٢٤ ساعة و٤٨ ساعة من العلاج. تم استخدام مقياس التناظرية البصرية (VAS) كمقياس للنتائج الأولية. شملت النتائج الثانوية القياس الكمي لكمیة الإنترلوكین- ٨ اللبي (نانوغرام/لتر) باستخدام ELISA بالإضافة إلى حدوث آلام ما بعد علاج الجذور.<br/>النتائج:<br/>أظهرت النتائج انه لم يكن هناك فرق كبير بين المجموعات الثلاث في متوسط العمر والجنس وتوزيع الأسنان. فيما يتعلق بكثافة الألم بعد المعالجة اللبية مباشرة، كان هناك فرق ذو دلالة إحصائية بين درجات الألم في المجموعات الثلاث حيث أظهرت مجموعة الدواء الوهمي أعلى درجة ألم ذات دلالة إحصائية ولا يوجد فرق بين مجموعتي التدخل: البريدنيزولون وديكلوفيناك البوتاسيوم. بالإضافة إلى ذلك، لم يكن هناك فروق ذات دلالة إحصائية بين درجات الألم في المجموعات الثلاث عند ٦ و١٢ ساعة. لم يبلغ أي من المرضى في المجموعات الثلاث عن ألم ما بعد علاج الجذور بعد ٢٤و ٤٨ ساعة. وعلاوة على ذلك، لم يوجد فرق ذو دلالة إحصائية من حيث حدوث آلام ما بعد علاج الجذور وبين مستويات وكمیة الإنترلوكین-٨ في المجموعات الثلاث.<br/><br/><br/>الاستنتاجات:<br/> ضمن حدود هذه الدراسة، خلصت الدراسة الحالية إلى أن مجموعة الدواء الوهمي لديها أعلى درجة من الألم من حيث شدة الألم بعد المعالجة اللبية مباشرة، ولم يكن هناك فرق بين مجموعتي البريدنيزولون وديكلوفيناك البوتاسيوم. لم يكن هناك فرق بين المجموعات الثلاث بعد ٦ و١٢ ساعة. لم يبلغ أي من المرضى عن الألم في المجموعات الثلاث بعد ٢٤ و٤٨ ساعة. بالإضافة إلى ذلك، لم يكن هناك اختلاف في حدوث الألم بعد علاج الجذور وكمية الإنترلوكين-٨ في المجموعات الثلاث. |
