| Summary, etc. | The aim of this study is to present a detailed comparison of <br/>outcome between Furlow's technique and rotational flap technique in <br/>primary repair of cleft palate, focusing on postoperative velopharyngeal <br/>competence. to achieve this goal 64 infants with cleft palate between 9 <br/>and 24 months were randomly assigned into 2 groups, group A Furlow <br/>and group B rotational flap and followed up in the paediatric surgery unit <br/>at Cairo University specialized paediatric hospital from February 2022 to <br/>February 2024. <br/>Our results showed no difference between both groups regarding <br/>immediate postoperative complications, rate of fistula and dehiscence was <br/>higher in group B but not statistically significant. Both techniques provide <br/>comparable outcomes in low-grade clefts regarding velopharyngeal <br/>competence. For higher-grade clefts, the addition of buccinator flaps with <br/>Furlow technique improves velopharyngeal valve competence with <br/>notable difference in palatal mobility and lateral pharyngeal wall mobility <br/>scores yet these differences were not statistically significant, however it is <br/>on the expense of statistically longer operative time. <br/>Nasoendoscopy is a valuable tool for VPI assessment however it is <br/>operator dependent and needs more objective standards. The study <br/>highlights the importance of early and appropriate intervention to <br/>minimize postoperative complications and need to refine surgical <br/>techniques and improve outcomes for patients with cleft palate, <br/>emphasizing the need for tailored approaches based on cleft severity and <br/>individual patient characteristics. |

| Summary, etc. | الهدف من الدراسة هو مقارنة تفصيلية لنتائج بين تقنية فورلو وتقنية السديلة<br/>الدوائية في الاصلاح الاولي للحنق المشقوق مع التركيز علي كفائة الصمام الحنكي البلعومي <br/>بعد عملية الجراخية و لتحقيق هذا الهدف تم تقسيم 64 رضيقا مصابابالحنك المشقوق تتراوح <br/>اعمارهم بين 9 – 24 شهرا بشكل عشوائي الي مجموعتين المحموعة (أ) تقنية فيرلو والمجموعة (ب) السديلة الدورانية، وتمت متابعتهم في وحدة جراحة الأطفال في مستشفي الأطفال التخصصي بجامعة القاهرة في الفترة من فبراير 2022 إلى فبراير 2024. أظهرت نتائجنا عدم وجود فرق بين المجموعتين فيما يتعلق بمضاعفات ما بعد الجراحة المباشرة ، وكان معدل الناسور والتفزر أعلى في المجموعة (ب) ولكن ليس ذات دلالة إحصائية. <br/>توفر كلا التقنيتين نتائج قابلة للمقارنة في الشقوق منخفضة الدرجة فيما يتعلق بالكفائة البلعومية. بالنسبة للشقوق ذات الدرجة الأعلى، تؤدي اضافة السدائل الخدية باستخدام تقنية فورلو الى تحسين كفاءة الصمام البلعومى بشكل ملحوظ مع إختلاف ملحوظ في الحركة الحنكية ودرجات حركة جدار البلعوم الجانبي، إلا أن هذه الخلافات لم تكن ذات دلالة إحصائية، ومع ذلك فهي على حساب وقت الأطول إحصائيا.<br/><br/>يعد تنظير الأنف أداة قيمة لتقييم كفاءة الصمام البلعومي إلا أنه يعتمد على المشغل ويحتاج الى معايير أكثر موضوعية. تسلط الدراسة الضوء على أهمية التدخل المبكر والمناسب لتقليل مضاعفات ما بعد الجراحة والحاجة الى تحسن التقنيات الجراحية وتحسين النتائج للمرضى الذين يعانون من الحنك المشقوق، مع التركيز على الحاجة الي أساليب مصممة خصيصا بناءا على شدة الشق والخصائص الفردية للمريض. <br/> |
