| Summary, etc. | Background: Stroke is a syndrome of rapidly developing clinical signs of focal or global disturbance of cerebral function with symptoms lasting 24 hours or longer or leading to death. Shoulder pain is one of the most common complications after stroke, affecting up to 70% of stroke patients. Purpose of the study: To investigate the effect of adding Maitland technique to specific proprioceptive neuromuscular facilitation (PNF) protocol on pain and shoulder range of motion (ROM), muscle power and function in patients with stroke shoulder pain. Subjects and Methods: Thirty patients with stroke shoulder pain were selected from the outpatient clinic of Faculty of Physical Therapy, Cairo University, and Kasr Al-ainy hospitals, Faculty of Medicine, Cairo University. Patients’ ages ranged from 45 to 60 years old. Spasticity of upper limbs ranged from (1 to 1+) (mild spasticity) according to modified Ashworth scale. Patients had Mini Mental State Examination (MMSE) score > 26. Patients had intact vision and hearing. Patients were randomly allocated to study and control groups. The study group received Maitland technique, specific PNF protocol and traditional physiotherapy program. The control group received the same specific PNF protocol and the traditional physiotherapy program only. Assessment procedures were visual analogue scale for shoulder pain intensity, isokinetic dynamometer for assessment of shoulder muscle power, functional independence measure (FIM) for evaluating functions of the shoulder and digital goniometer for measurement of shoulder range of motion (ROM). All measures were done before and after treatment sessions which were conducted three times per week for 12 successive weeks. Results: There was a significant increase in the range of motion of shoulder joint, improvement of muscle power, and function of the shoulder and a significant decrease in shoulder pain in both groups post treatment with the study group showed more significant improvement compared to the control group. Conclusion: Maitland technique is effective and beneficial when added to proprioceptive neuromuscular facilitation and traditional physical therapy protocol in the rehabilitation of patients with stroke shoulder pain. |

| Summary, etc. | الخلفية: السكتة الدماغية هي متلازمة التطورالسريع لعلامات سريرية نتيجة إضطراب بؤري في وظيفة المخ مع أعراض تستمر 24 ساعة أو أكثر او تؤدي الي الوفاة دون سبب واضح بخلاف الأصل الوعائي .<br/>هدف الدراسة: إختبار تأثير إضافة تقنية ميتلاند الي بروتوكول التيسير العصبي العضلي المحدد علي المرضي الذين يعانون من آلام الكتف نتيجة سكتة دماغية.<br/>الأشخاص والأساليب: تم إختيار ثلاثين مريضا يعانون من آلام الكتف نتيجة سكتة دماغية من العيادة الخارجية لكلية العلاج الطبيعي، جامعة القاهرة، ومستشفي القصر العيني، كلية الطب، مصر. تراوحت أعمار المرضي بين 45 الى 60 عاما. كانت درجة تشنج الطرف العلوي بين (1 الي +1) وفقا لمقياس أشوورث المعدل. تم تقسيم المشاركين بشكل عشوائي إلي مجموعتين: مجموعة دراسة ومجموعة ضابطة. تلقت مجموعة الدراسة تقنية ميتلاند بلإضافة إلي بروتوكول التيسير العصبي العضلي المحدد وبرنامج العلاج الطبيعي التقليدي. بينما تلقت المجموعة الضابطة نفس بروتوكول التيسير العصبي العضلي المحدد وبرنامج العلاج الطبيعي التقليدى فقط. خضع جميع المشاركين للتقييم بإستخدام مقياس التناظر البصري، مقياس القوة، مقياس الزوايا الرقمي ومقياس الإستقلال الوظيفي.<br/>النتائج: أظهرت نتائج البحث وجود تغيرات ذات دلالة إحصائية في مرضي المجموعتين بعد العلاج في صورة نقص آلام الكتف و زيادة حركة مفصل الكتف و قوة العضلة ووظيفة الطرف العلوي. و قد كان التحسن أكبر بفارق إحصائي في مجموعة الدراسة مقارنة بالمجموعة الضابطة.<br/>الخلاصة: إضافة تقنية ميتلاند إلي بروتوكول التيسير العصبي العضلي المحدد له تأثيرفعال علي الألم و حركة مفصل الكتف و النتاج الوظيفى للطرف العلوي في المرضي الذين يعانون من آلام الكتف نتيجة السكتة الدماغية.<br/> |
