| Summary, etc. | Background: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease that can involve any organ system and may lead to significant morbidity and even mortality. Prolactin is a polypeptide hormone has a heterogeneous function, it has a recognized immunostimulatory effect, especially inhibiting the negative selection of autoreactive B lymphocytes, promoting autoimmunity.<br/>Aim of work: The present work was conducted to assess changes at serum prolactin levels among children and adolescents diagnosed as systemic lupus and it's relation to disease activity.<br/>Patient and method: This was a cross sectional analytic study conducted at Cairo University Children Hospital, on Patients who were diagnosed as having systemic lupus aged 5 years or more and are being followed up at the Pediatric Rheumatology Clinic. It included 40 cases with SLE and 40 age and sex matched children were taken as control. <br/>Results: The SLE group mean age was 11.5±2.1 years and there was female predominance as 85% of our patients were females. The main clinical manifestations in our study were arthritis (67%) followed by malar rash (65%), while the less frequent manifestation was Discoid rash (5%), Renal Disorders (57.5%), Hematological disorders (47.5%) and Neurological disorders (17.5%). In the current study, AST, ALT, BUN and ESR levels were significantly higher in SLE group than in control group. In the present study, regarding anti-double strand antibodies (ADNA) were positive in (75%) of SLE group while anti-nuclear antibodies (ANA) were positive in (95%). In our study, regarding to disease severity; the disease was Moderate in (45.0%), Severe in (37.5%) and mild in (17.5%). prolactin level was significantly higher among SLE cases with mean was17.3±6.6 than controls as its mean was 13.5±5.3 in controls, it considered high than normal level among 8 (20.0%) (2 moderate and 4 sever) cases in compare to no cases reported in controls. Cases with Neurological disorders significantly had higher prolactin level. However, we did not find significant difference between hyperprolactinemia regarding any drug features (p > 0.05).<br/>Conclusion: Prolactin levels were significantly higher in children with SLE patients than in controls; furthermore, High prolactin levels may play a role in disease activity as a significantly positive correlation was existed between prolactin level and SLE severity score. |

| Summary, etc. | الذئبة الحمراء هو مرض مناعى ذاتى من ضمن الامراض الروماتيزميه الأكثر شيوعًا عند النساء من الرجال وتبلغ ذروته خلال سنوات الإنجاب سببه غير معروف و يتميز بوجود اجسام مضاده واعراضه تؤثر على اجهزه عديده بالجسم. بالدراسه تبين أن عشرين في المائة إلى ثلاثين في المائة من الأشخاص المصابين بمرض الذئبة الحمراء وخاصة أولئك الذين يعانون من مرض نشط يعانون من فرط برولاكتين الدم الخفيف إلى المتوسط.<br/>تشير الأرقام أن عدد النساء المصابات بمرض الذئبة الحمراء أكثر من الرجال و هذا يشير إلى دور العوامل الجنسية في التأثير على قابلية ظهور المرض وتطوره. فكرة أن الهرمونات الجنسية تعدل حدوث وشدة المرض لدى مرضى الذئبة الحمراء مدعومة بأدلة قوية على وظيفة البرولاكتين فى تحفيز المناعه.<br/>يتم تعزيز المناعة عن طريق البرولاكتين ، وهو هرمون متعدد الببتيد يتكون من 199 حمض أميني بوزن جزيئي يبلغ 23 كيلو دالتون. من بين وظائفه العديدة فأن البرولاكتين له تأثير محفز للمناعة ، لا سيما عن طريق منع الاختيار السلبي للخلايا اللمفاوية البائية النشطة. تكون مستويات البرولاكتين في الدم أعلى بشكل ملحوظ في المرضى الذين يعانون من مرض نشط مقارنة بالمرضى الذين يعانون من مرض غير نشط. يتناسب مستوى الأجسام المضادة للحمض النووي المزدوج الشريطية عكسياً مع تركيزالبرولاكتين كما ترتبط مستويات البرولاكتين فى الدم بالنشاط السريري والمصلى للمرض .و يعانى المرضى الذين لديهم ارتفاع نسب البرولاكتين التهاب الكلية الذئبي النشط ، وكذلك التعب أو الحمى. وتشير الأبحاث إلى أن مستوى البرولاكتين يلعب دورًا مستمرًا في الالتهاب.<br/>هدفنا في دراستنا هو تقييم مستوى هرمون البرولاكتين لدى الاطفال و المراهقين الذين يعانون من مرض الذئبه الحمراء وعلاقته بنشاط المرض.<br/>قمنا بتضمين المرضى الذين تم تشخيص إصابتهم بمرض الذئبة الحمراء وفقًا لمعايير الكلية الأمريكية لأمراض الروماتيزم مع استبعاد المرضى الذين يعانون من أمراض الروماتيزمه الأخرى والمرضى الذين يعانون من أمراض أخرى قد تؤثر على مستوى البرولاكتين في الدم. |
