| Summary, etc. | Background; Acetabular fractures are associated with a high rate of late post-traumatic degenerative changes, as much as (57%) in some reports. THA can be performed after an acetabular fracture, to manage secondary degenerative change or, possibly, osteonecrosis of the femoral head. After initial nonsurgical treatment of an acetabular fracture, an occult or frank acetabular nonunion and or malunion are not uncommon and may extend to the residual pelvic ring deformity. Aim and objectives; Evaluation of the functional and clinical outcomes of total hip replacement which are done for management of acetabular fractures complications as arthritis due to malunion and or AVN by both Harris Hip score and WOMAC score and prevent the most common complications as infection, dislocation, heterotopic ossification and aseptic loosening following THR for management of acetabular fractures complications. Subjects and methods; This was a case series (prospective) study included 30 patients with THA for management of acetabular fractures complications as arthritis due to malunion and or AVN. This study started in November 2021 till August 2023. Result; A statistically significant difference was found between preoperative and postoperative functional outcomes using Harris Hip Score and WOMAC score (Paired sample t test, P = .001). A statistically significant improvement was found in terms of HO after use of ketorolac at 18-months follow-up compared to preoperatively (Chi-square test, P = .001) , also 2 cases of prosthetic dislocation , 2 cases of cup loosening and no case of infection was reported.<br/>Conclusion; Total hip replacement was safe and effective in the management of complicated acetabular fractures. Ketorolac is effective in preventing HO after THR , with less complications in comparison to results of indomethacin and radiation in the literature.The current study was limited by small sample size, the lack for control group and relatively short follow up period. |

| Summary, etc. | كسور الحوض ترتبط بمعدل عالٍ من التغيرات التنكسية المتأخرة بعد الإصابة، يصل إلى 57% في بعض التقارير. قد ينتج تدهور مفصل الفخد عن رد غير مثالي للكسور، وإصابات الغضروف في حق مفصل الفخد، وإصابات رأس عظم الفخذ، و التهاب المفاصل الموجود مسبقًا. تنخر الغضروف قد يكون نتيجة الأضرار التي يسببها عدم اكتشاف تواجد المسامير داخل المفصل، أو بسبب العدوى الميكروبية، أو نتيجة الإصابة الأولية للغضروف المفصلي. تم تسجيل مرض نقص الدورة الدموية وتنخر رأس عظمة الفخذ بعد كسور الحوض بنسبة تتراوح بين 2% إلى 10%، ويمكن أن يقلل فوائد التثبيت التشريحي الدقيق مع تثبيت مستقر ونطاق حركة مبكر . <br/>بعد العلاج التحفظي الأولي لكسور الحوض، فإن وجود الٍتئام غير ظاهر أو واضح لعظمة حق مفصل الفخذ و/أو تشوه الحوض, ليس نادرا وقد يمتد إلى تشوه باقي هيكل الحوض. بعد العلاج الجراحي، يمكن مواجهة مخاطر إضافية مثل وجود معدن صلب داخل المفصل ، تشكل عظمي خارج موضعه، نسيج ندبة كثيف، عضلات أو عظام تعاني من نقص في الدورة الدموية، وعدوى غير ظاهرة.<br/>يمكن إجراء استبدال كلي لمفصل الفخذ بعد كسور الحوض لعلاج التغير التنكسي الثانوي أو ربما، نخر العظام في رأس عظمة الفخذ.<br/>التحوّر العظمي غير المنتظم خارج موضعة هو مضاعفة شائعة نسبيًا يتم اكتشافها في حوالي 30% من الحالات، وتُصنَّف وفقًا لتصنيف بروكر، يتم إجراء صور أشعة سينية قبل العملية الجراحية لاكتشاف وجود التحوّر العظمي، وينتظر حتى ينضج التحوّر العظمي لمنع عودته، من خلال استخدام التصوير النووي والبحث عن تناقص في امتصاص المادة المشعة عند مقارنة نتائج اختبارات التصوير الوميضى.<br/>لتقليل حدوث التحوّر العظمي غير المنتظم خارج موضعة تم اعطاء المريض الكيتورولاك 60 مجم اثناء العملية جرعة واحدة ثم يعطي بجرعة 30 مجم بعد العملية كل 8 ساعات عن طريق الحقن . |
