Summary, etc. Introduction: Foundation of Total knee replacement surgery is based on three pillars soft tissue balancing, bone cuts and cementing. All these factors are equally important and mistake at any level will lead to failure. We would like to emphasize the importance of “soft tissue role in Joint line Restoration” as a tool for successful 1ry TKA. The objective of this current study is to measure joint line, restore mechanical axis, and restoration of patella-femoral alignment, balanced flexion and extension gaps by balancing of soft tissue around the knee. In addition, we tried to collect evidence for the restoration of joint line by maintenance of the soft tissue around the knee and its radiological and functional consequences on our patients .<br/> Patients and Methods: The current study include 43 knees (42 patients) underwent TKA by balancing the extension gap before the flexion gap. Radiographic evaluation included measurements of the joint line from the fibular head and from the adductor tubercle in the antero-posterior view, while measuring the Insall-Salvati ratio and Caton-Deschamps ratio in the lateral views. The functional assessment was done using Range of motion before and after TKA and Oxford-Knee-Score.<br/> Results: 42 patients were followed pre-operatively and 1,3,6 months post-operatively .The mean age was 61.81± 3.54. There was marked improvement and statistically significant difference between pre and one year post-operative functional scores. Regarding the Oxford-Knee-Score the mean score was 10.09 ± 2.98 before knee replacement, and became 46.67 ±1.32 after 6 months with P value <0.001. Radiologically, however, it did not show significant difference. The mean of (Joint line distance measured from the fibular head) was 16.2 mm ±7.4 pre-operatively and became 16.8 ± 5 after 6 months post-operatively. The mean of (Joint line distance measured from the adductor tubercle) was 39.5 mm ± 9.01 pre-operatively and became 40.1a ±8.5. Regarding the complications, 7% of our patients had minimal post-operative complications.<br/> Conclusion: Using the gap balancing technique as guidance for an equilibrium way among soft tissues surrounding the knee during primary total knee replacement helps in maintaining joint line to almost normal ranges following the knee replacement. It has an significant effect on clinical response of patients undergoing such operations specially their range of motions.

Summary, etc. ان اساس عملية تغير مفصل الركبة الاولية هو الاعتماد على توازن الانسجة الخارجية المحيطة بالركبة وحواف عظم الركبة المقطوعة والاسمنت الطبى الموضوع بالركبة.ونحن ننوه فى هذه الدراسة الى العنصر الرابع وهو ضرورة المحافظة على سطح المفصل بعد تركيب المفصل الصناعى.لذلك كان هدفنا الاساسى فى هذه الدراسة هو اثبات ان المحافظة على توازن الانسجة الخارجية المحيطة بالركبة تحافظ على مستوى سطح المفصل الى ماكان عليه قبل تركيب المفصل.<br/>ولقد قمنا باجراء 43 عملية تغير لمفصل الركبة ل 42 مريض اعتمادا على المحافظة على توازن الانسجة المحيطة بمفصل الركبة كهدف اساسي.وقمنا بواسطة اشعة اكس بقياس سطح مفصل الركبة قبل وبعد العملية على فترات 1و3و6 اشهر وذلك من نقطتين ثابتين على الاشعة وهما رأس عظمة الشظية والحديبة المقربة لعظمة الفخذ. كما تم قياس معدل انحراف عظمة الردفة قبل وبعد العملية.وأخيرا تم تقييم شكوى المريض الاكلينيكية قبل وبعد العملية على نفس فترات قياس سطح المفصل.<br/>وعند تحليل النتائج احصائيا تبين عدم وجود تغيير محلوظ بسطح المفصل بعد تركيب مفصل الركبة وعدم وجود تغيير ملحوظ فى حركة عظمة الردفة .وعلى العكس تبين تحسن كبير فى شكوى المريض بعد العلمية مع رجوع معظم المرضى الى ممارسة الأنشطة اليومية المعتادة لهم. مع حدوث حوالى7% كنسبة مضاعفات انتهت بعد فترة قصيرة من العملية.ولقد توصلنا بعد فحص النتائج الى ان الاعتماد على توازن الانسجة المحيطة بمفصل الركبة اثناء عملية تغير مفصل الركبة الاولية تحافظ على ثبات سطح مفصل الركبة الى تقريبا نفس مستواه قبل العملية.بالاضافة الى تحقيق نتائج اكلينيكة مرضية جدا لمعظم المرضي وسرعة عودتهم الى حياتهم العملية قبل تغير مفصل الركبة.
