Summary, etc. Background: A growing body of evidence indicated that vitamin D has a potential protective role against neurovascular injury. Specifically, low serum 25(OH) Vitamin D has been associated with increased risk for incident stroke and stroke fatality. Exact causality is however not yet established. Objectives: This study aimed to explore vitamin D status among acute ischemic stroke patients and its relation to initial severity and short-term outcome of acute ischemic stroke. Subjects and methods: The study was carried on forty-eight acute ischemic stroke patients and forty-eight matched healthy control subjects. Serum vitamin D was done for all patients and control subjects. National Institute of Health Stroke Scale (NIHSS) on admission and after 72 hours and modified Rankin Scale (mRS) on discharge and after 3 months were performed for all patients. Measurement of serum vitamin D was done for all patients and control subjects. Serum C Reactive Protein (CRP) and calcium were also done for all patients. Brain computed Tomography (CT) scan was also done for all patients and the volume of infarction was obtained. Carotid and vertebrobasilar duplex was done for thirty patients. Results: There was a statistically highly significant difference between patients and controls regarding serum vitamin D levels being lower in the patients group. Vitamin D deficiency and insufficiency were also significantly prevalent among stroke patients compared to healthy controls. A statistically significant difference was detected between patients{u2019} subgroups distributed according to vitamin D status regarding mean age. Hypertension was more prevalent in the deficient subgroup. Patients with deficient vitamin D had more severe stroke and worse short term outcome compared to other subgroups
