Summary, etc. Background: Hypertensive disorders affect about 10% of all pregnancies worldwide contributing a major cause of maternal and perinatal morbidity and mortality. Up to 10{u2013}15% of maternal deaths & 20 % perinatal mortality are directly associated with pre-eclampsia and eclampsia in developing countries. The (HELLP) syndrome is a serious complication in pregnancy characterized by hemolysis, elevated liver enzymes and low platelet count occurring in 0.4 to 0.7% of all pregnancies and in 10-12% of cases with severe preeclampsia. This study will throw some light on difference between postpartum maternal and neonatal outcome between both severe preeclampsia and HELLP syndrome associated with abdominal ascites groups. Methods: A prospective observational study was conducted at Department of Obstetrics and Gynecology, Kasr El-Ainy University Hospital, Cairo University, from September 2018 {u2013} March 2019. It included One Hundred and Thirty pregnant women Diagnosed with hypertension in the current pregnancy complicated with severe Preeclampsia, or HELLP Syndrome associated with abdominal ascites, recruited from the attendees of the Obstetric Emergency Department. The study was approved by the ethical committee of the Faculty of Medicine, Cairo University. All patients were counseled about the procedure and a verbal informed consent was obtained from all cases
