The use of dental implant in the restoration of missing teeth became a routine treatment modality due to its high success rates and proven biological and mechanical advantages. Crestal bone resorption and peri-implantitis are considered amongst the main biological and esthetical complications challenging the long term success of dental implants. This crestal bone resorption can be attributed to the lack of periodontal ligaments at the bone-implant interface with subsequent stress concentration on loading with the superstructure.
Recently with the development of new materials such as Polyetheretherketone (PEEK), a polymer having lighter weight than conventional framework materials and modulus of elasticity comparable to bone can significantly decrease the stresses falling on the underlying implants thus increasing the survival and clinical performance of dental implants.
The aim of the current study was to evaluate patient`s satisfaction, gingival reaction and crestal bone resorption around dental implants restored with customized PEEK abutments compared to Zirconium abutments and loaded with PEEK superstructure.
30 implants in the esthetic were restored with custom made hybrid abutments made from PEEK and zirconium both groups were loaded with PEEK crowns. Patient`s satisfaction, gingival reaction and crestal bone resorption were assessed for 1 year.
There was no difference between PEEK and zirconium abutments regarding patient`s satisfaction, clinical performance, gingival reaction nor crestal bone loss, although PEEK abutments showed slightly improved soft tissue response compared to zirconium.

It was concluded that PEEK is a viable substitute to zirconium abutments when restoring dental implants, with the advantage of decreasing plaque accumulation. PEEK crowns decreases the forces falling on the dental implants due to their relatively low modulus of elasticity that is comparable to bone.

أصبح استخدام زراعة الأسنان في تعويض الأسنان المفقودة طريقة علاج روتينية بسبب معدلات نجاحها العالية ومزاياها البيولوجية والميكانيكية المثبتة. يعتبر تأكل العظم والتهاب الأنسجة المحيطة الزرع من بين المضاعفات البيولوجية والجمالية الرئيسية التي تتحدى النجاح طويل المدى لزراعة الأسنان. ويرجع تأكل العظم إلى عدم وجود الأربطة الداعمة للسن عند التقاء الزراعة مع العظام مما يؤدي إلى تركيز الضغط عند التحميل على التركيبات المثبتة على الزرعات.
في الآونة الأخيرة مع تطوير مواد جديدة مثل Polyetherketone (PEEK) ، المكون من البوليمر و الذي يكون وزنه أخف من مواد الهيكل التقليدية ومعامل المرونة الخاص به يكون مقارب بالعظام بشكل كبير مما يقلل من الضغوط الواقعة على الغرسات الأساسية وبالتالي زيادة مدة البقاء والأداء السريري لزراعة الأسنان .
كان الهدف من الدراسة الحالية هو تقييم رضا المريض وتفاعل اللثة و تاكل العظم حول زراعة الأسنان المحملة بدعائم PEEK مقارنة بدعامات الزركونيوم والمحملة ببنية PEEK الفوقية.
تم التحميل على 30 غرسة في المنطقة الأمامية ا باستخدام دعامات هجينة مصنوعة خصيصًا من PEEK والزركونيوم تم تحميل كلتا المجموعتين بتيجان PEEK . تم تقييم رضا المريض وتفاعل اللثة وتاكل العظم لمدة سنة واحدة من التحميل.
لم يكن هناك فرق بين دعامات PEEK والزركونيوم فيما يتعلق برضا المريض ، والأداء السريري ، ورد فعل اللثة أو فقدان العظام القشرية ، على الرغم من أن دعامات ال PEEK أظهرت تحسنًا طفيفًا في استجابة الأنسجة الزراعة مقارنة بالزركونيوم.
استنتج أن PEEK هو بديل قابل لللإستخدام بدلا من دعامات الزركونيوم عند التركيب على غرسات الأسنان ، مع ميزةتقليل تراكم الباكتيريا . تيجان ال PEEK تقلل الحمل على غرسات الأسنان بسبب معامل المرونة المنخفض نسبيًا الذييمكن مقارنته بالعظام.
