Background: Anticoagulation therapy is most commonly indicated in the presence of atrial fibrillation, deep venous thrombosis, and after placement of prosthetic heart valves. It can cause many complications, which can be prevented. Aim of the study: To evaluate the effect of health regiment guidelines about oral anticoagulant therapy on the patients' post operative complication. Research design: A quasi experimental study research design was utilized in the current study Research Hypotheses: The patient who will exposed to the health regiment guidelines about anticoagulant therapy will have lesser post-operative complications, than those who don’t exposed, and the patient who will exposed to the health regiment guidelines will have high knowledge level than those who don’t. Sample: A purposive sample of 171 adult male and female patients on oral anticoagulant after valve replacement surgery. Tools of data collection: Three tools were utilized to collect data pertinent to the current study: Structured Interview Questionnaire Sheet, Medical Data Sheet, and Monitoring of INR by Marevan Therapy Chart. Results: (33.3%) of participants were raised between 50<60 years old with the mean age37.89 ± 10.47. Most of patients were married (90.6%) concerning patient's genders, more than half were females (53.2%). the patient’s showed higher mean knowledge scores in posttest when compared to pretest except knowledge about follow up of anticoagulants. that no significant statistical difference in INR level by their medical data (posttest). Conclusion: There is a highly significant statistical difference in knowledge scores among patients before and after the conduction of health regiment guidelines. So, more than half of patients become aware of the benefits and complications of anticoagulants therapy in posttest when compared to pretest. Recommendations: Health education program about health regiment guidelines about oral anticoagulant and its effect on post-operative complications should be provided for post-operative patients to improve their health status and prevent complications, Replication of the same study on larger probability sample at different geographical locations for data generalizability

تعد جراحات تغيير صمامات القلب من أهم الجراحات التى تجرى فى كبرى المستشفيات والتى يكون سببها ضيق شديد فى الصمام أو قصور مما يؤدى إلى تدفق الدم ببطء أو تدفقه فى الأتجاه المعاكس، مما يصعب على القلب أداء وظيفته بشكل سليم.ولذلك تهدف عمليه تغيير صمامات القلب لزرع صمام قلب جديد صناعى أو بيولوجى، مكان الصمام المصاب ,وللحفاظ عليه يجب على المريض أن يخضع لأدويه مضاضات التجلط ( الماريفان ) الذى يقلل من تكوين جلطات الدم عن طريق منع تكوين بعض عوامل التخثر ( التجلط ) التى تتنشط بوجود فيتامين ك . ولابد من أخذ بعض الأحتياطات بعد الجراحه للحفاظ على الصمام أن يقوم المريض بعمل فحوصات دوريه لقيمه نسبه السيوله الدوليه ( INR ) وذلك أمر ضرورى فهناك عوامل مثل النظام الغذائى، والأدويه، النباتات، الأختلافات الجينيه فى الإنزيمات قد تؤثر على أستجابه المرضى للورفارين. (الماريفان ).

ولذلك يأتى دورنا كهيئة التمريض فى تقديم النصح والارشادات للمرضى وذلك بتقديم نظام صحى يتصف بضرورة الامتثال لجدوله جرعات مضادات التجلط المنصوص عليها فى العلاج، عدم التوقف المفاجئ بدون أى أسباب، وعدم أخذ أى أدويه مثل المسكنات بدون أستشاره الطبيب، الحفاظ على النظام الغذائى المتوازن وتجنب بعض المأكولات والعصائر، والقيام بزيارات منتظمه الى الطبيب أو العيادات الخارجيه لمتابعه التحاليل والعلاج، ووذلك لمنع وتقليل المشاكل التى قد تحدث من عدم الألتزام بهذه التعليمات والنصائح.
