Objective: The aim was to compare the effect of arm ergometer to the effect of functional electric stimulation on upper extremity strength and function in children with unilateral cerebral palsy who underwent surgical correction and those who didn’t. Subjects: sixty children (thirty two boys and twenty eight girls) with unilateral cerebral palsy whose age ranged from five to eight years participated in this study. They were classified randomly into four groups of equal number; fifteen patients received the arm ergometer training group without wrist surgical intervention, fifteen patients in the arm ergometer training group after wrist surgical intervention, fifteen patients in functional electric stimulation group without wrist surgical intervention and fifteen patients in functional electric stimulation group with wrist surgical intervention. All groups received selected physiotherapy program for sixty minutes, three times per week along ten weeks. Methods: Isometric strength of the affected upper extremity using Lafayette hand held dynamometer and quality of upper extremity skills test were measured for all four groups before and after ten weeks of treatment. Results: A significant improvement was recorded in the measured variables in the four groups when comparing their pre and post treatment mean values of muscle strength and QUEST scores within groups. ANOVA analysis conducted to compare four groups revealed significant improvement of protective extension in surgical group that received arm ergometer. Conclusion: From the obtained results of this study, it could be concluded that both functional electrical stimulation and arm ergometer are beneficial to improve upper limb strength and functional outcomes. Further research work needed to study effect of interventions post-surgical correction on longer period of time

الغرض من هذه الرسالة: هو مقارنة تأثير العجلة للذراعين بتأثير التحفيز الكهربائي الوظيفي على قوة الطرف العلوي ووظيفته لدى الأطفال المصابين بالشلل الدماغي الأحادي الجانب سواء خضعوا للتصحيح الجراحي أو لم يخضعوا له. الاشخاص: شارك في هذه الدراسة 60 طفلاً (32 ولدًا و28 فتاة) مصابين بالشلل الدماغي الأحادي الجانب وتراوحت أعمارهم بين خمس إلى ثماني سنوات. وتم تصنيفهم عشوائياً إلى أربع مجموعات متساوية العدد؛ تلقى 15 مريضًا مجموعة التدريب على عجلةالذراعين دون تدخل جراحي، و15 مريضًا في مجموعة التدريب على عجلةالذراعين بعد التدخل الجراحي للمعصم، و15 مريضًا في مجموعة التحفيز الكهربائي الوظيفي دون تدخل جراحيم، و15 مريضًا في مجموعة التحفيز الكهربائي الوظيفي مع تدخل جراحي للمعصم. كما تلقت جميع المجموعات برنامج العلاج الطبيعي المحدد. الطرق: تم قياس قوة عضلات للطرف العلوي المصاب باستخدام مقياس قوة لافاييت المحمول واختبار جودة مهارات الطرف العلوي لجميع المجموعات الأربع قبل وبعد عشرة أسابيع من العلاج. النتائج: تم تسجيل تحسن كبير في المتغيرات المقاسة في المجموعات الأربع عند مقارنة قيم متوسط قوة العضلات قبل وبعد العلاج ودرجات QUEST داخل المجموعات. كشف تحليل أنوفا الذي تم إجراؤه لمقارنة 4 مجموعات عن تحسن كبير في الامتداد الوقائي في المجموعة الجراحية التي تلقت تدريب العجلة للذراعين. الاستنتاج: من النتائج التي تم الحصول عليها من هذه الدراسة، يمكن استنتاج أن كلا من التحفيز الكهربائي الوظيفي وتدريب العجلة للذراعين مفيدان لتحسين قوة الطرف العلوي والنتائج الوظيفية خاصة بعد العمليات الجراحية التصحيحية.
