The main objectives of the study: 1- to subjectively compare the stability of two different designs of custom-made plates used for the fixation of the maxilla following LeFort1 osteotomy to best maximize the benefit of Virtual Surgical Planning (VSP) and the use of Patient Specific Implants (PSIs). 2- Take a further step forward in optimizing the best virtual surgical workflow from planning to execution to maximize the benefits of computer guided surgery. Materials and methods: A Randomized clinical trial was done to compare the stability of the operated maxilla following orthognathic surgery. 34 patients were recruited from the Oral and maxillofacial surgery clinic and were divided into two groups: the first group a one-piece fixation plate was used, while a two-piece fixation plate was used in the second group. Immediate and four moths postoperative CT scans were done to evaluate the stability using seven dental and bony landmarks. Summary of the results: There were no statistically significant differences (P ≤ 0.05) between the two groups in all seven landmarks used for stability measurements, indicating the absence of significant clinical stability between the two groups and the two different fixation schemes used. Primary conclusion: Based on this research findings, the two different fixation schemes were proved to be very similar after 4 months of surgery. Further longer-term studies are indicated to reach final conclusions about the use of PSIs and the best use of VSP in orthognathic surgery.

المقارنة ما بين ثبات تصميمين مختلفين للشرائح مخصوصة الصنع للمريض لتثبيت الفك العلوي في الجراحات التقويمية وذلك لتحقيق أكبر استفادة للمرضي من تقنيات الجراحات الافتراضية. المواد والأساليب تم عمل تجربة سريرية عشوائية لمقارنة تصميمين مختلفين من الشرائح مخصوصة الصنع للمريض. تم احراء الجراحة على 34 مريض مقسمين الي مجموعتين متساويتين. في المجموعة الاولي تم استخدام شريحة واحدة بطول الفك العلوي وفي المجموعة الثانية تم استخدام شريحتين. ثبات الفك العلوي بعد الجراحة في المجموعتين تم مقارنته باستخدام الاشعة المقطعية بعد الجراحة بأسبوع وأربعة شهور باستعمال سبعة نقاط مختلفة في الفك العلوي ملخص النتائج تم العثور على فرق غير معنوي إحصائيا بين المجموعتين في كلا من السبعة نقاط المحددة على الاشعة المقطعية في المجموعتين. ذلك يشير على عدم وجود اختلاف في ثبات الفك بعد أربعة شهور من الجراحة في المجموعتين
الاستنتاج بناء على نتائج هذا البحث، لا يوجد فرق ما بين التصميمين المختلفين للشرائح مخصوصة الصنع للمريض. ينصح عمل ابحاث اخري للتوصل تلي أفضل نتائج لاستخدام تكنولوجيا الجراحة الافتراضية والطباعة ثلاثية الابعاد
