Its mechanism of action involves inhibition of PDE3, leading to elevation of cAMP, increasing the entry of calcium ions into the cardiac tissues, and hence increasing the contractility of the cardiac muscle. Moreover, recent studies showed that PDE3 and other PDEs are over-expressed in malignant tumors and inhibition of such PDEs and consequent high levels of cAMP, or cGMP, or both may be associated with anticancer and apoptotic activities

Milrinone is a 3-cyano-2-pyridone derivative that has been introduced for the treatment of CHF.
