This study suggests that fixation of unstable medial four metacarpal fractures results in predictable satisfactory radiographic and functional outcome . Also it suggests that percutaneous K wires insertion carries no risk of damage to soft tissues and painful open surgeries . Also using this technique is easy to achieve and maintain reduction although the procedure easy to interpert the fluoroscopy images and it is easy to insert the K wires in different techniques
