This thesis, reviewed the anatomy of the cranial vault, the causes of skull defects, and the indications, contraindications and aims of repair of skull defects. We discussed the physiological importance of the integrity of the skull bones and different methods of repair of cranial vault defects regarding the maintenance of optimum blood flow and cerebral perfusion. This study included 25 cases with cranial defects requiring repair, caused by different etiologies, and demonstrated the results. These cases have been operated upon by either placing an alloplastic graft (methylmethacrylate or titanium mesh) or by replacing the patient's own bone fragments in early post-traumatic cases. Results have shown the least infection rates with the use of the patient's own bone grafts and the highest when using methylmethacrylate that is not impregnated with antibiotics. When comparing methylmetacrylate with titanium mesh, methylmethacrylate has the advantage of being more malleable and more readily available while titanium mesh is more resistant to fracture
