Lichen planus is a dermatological disease affecting skin and / or mucous membranes. Methylene tetrahydrofolate reductase gene polymorphism and its accompanying hyperhomocysteinemia may contribute to the pathogenesis or the prognosis of many diseases including lichen planus. The aim of this study is to detect MTHFR (C677T) polymorphism, and to find its association with homocysteine and folic acid levels in patients with lichen planus. This study was done on 55 patients of lichen planus and 30 healthy controls classified into three groups, group 1: including 30 control healty subjects, group 2: 20 oral lichen planus cases and group 3: 35 cutaneous lichen planus patients. MTHFR gene polymorphism was done by PCR-RFLP. Serum levels of homocysteine and folic acid were measured by ELISA. Patients with lichen planus showed three genotypes: TT, CT and CC genotype, the mutant TT genotype was statistically significant in oral lichen planus cases and in cutaneous lichen planus cases compared to control subjects, also there was an increase in homocysteine and decrease in folic acid levels in both patients' groups compared to control group
