Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Orthopedic Surgery Summary: For all these reasons, the use of either technique should be decided on the basis of the patient's age and type of work, and on the features clinical and radiological of the arthritic process. In the presence of concomitant peri-trapezial OA, arthroplasty is generally recommended. The joint is composed of the trapezium and the first metacarpal. it{u2019}s a hinge joint that lacks bony constrains so it depends on the ligaments for its stability. There is sixteen different ligaments, the most important is the volar peak ligament.Of all the techniques considered, the ones most used, and for which there exist retrospective studies evaluating their long-term effectiveness, are arthrodesis and arthroplasty. The advantages of arthrodesis are that it reduces the painful symptoms and ensures recovery of the stability of the base of the thumb and of gripping force, thereby leaving the individual the possibility of performing heavy work. However, it requires immobilization in plaster followed by a period of rehabilitation for complete recovery of function. It also leads to limitation of the mobility of the thumb, especially in retropulsion, and thus to a decrease in manualdexterity
