Background: Sepsis is a major challenge in ICU and it is associated with high mortality. Early diagnosis and quantification of its severity will allow appropriate management and may help in improving outcome. Many researchers have focused on factors that predict mortality in sepsis and many biomarkers have been postulated as predictors of outcomes.Copeptin, the C-terminal fragment of arginine vasopressin (AVP), has been suggested as stress hormone and a stable surrogate marker for the unstable AVP. Objective: The work will study the relation between Copeptin and the outcome in patients with sepsis in terms of mortality as primary end point and multiple organ failure as secondary outcome . Results: copeptin can be a biomarker for severity and outcome in patients with sepsis and sensitive predictor of mortality and MOF development. Correlation of copeptin with creatinin concentration may identify patients with sepsis who develop renal impairment and Copeptin can diagnose sepsis complicating cardiac patients and help improve their outcome. Conclusion: copeptin can be a biomarker for severity and outcome in patients with sepsis and sensitive predictor of mortality and MOF development
