The aim of the was to evaluate the role of MRI and US in the provisional diagnosis of uterine and adnexal malignant and surgically difficult cases, comparing their results with surgical evaluation and finally with the pathological examination as a gold standard of diagnosis. Thirty cases with uterine malignant and surgically difficult disorders and thirty cases with adnexal malignant and surgically difficult disorders were all under gone both US and MRI imaging and results for were compared with surgical evaluation and finally with pathology. US results matched pathological exam in 22 out of 30 uterine cases 73.3% and MRI matched only in 13 cases out of 30 cases 43.3% with significant difference (P. value 0.040), but MRI matched pathological results in 24 out of 30 adnexal cases 80% and US matched only in 16 cases 53.3% with significant difference, (P. value 0.044). The results of our study showed the superiority of US over MRI in provisional diagnosis of uterine malignant and surgically difficult cases however MRI had the upper hand in adnexal cases
