Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Oral and Dental Medicine - Department of Oral and Maxillofacial Surgery Summary: A variety of injectable corticosteroid , hyaluronic acid and NSAIDS formulations are used to reduce the persistent pain associated with TMJ destruction. Localized drug delivery via intra-articular injections minimizes ectopic effects while alleviating joint pain and other symptoms.Current methods of intra-articular drug delivery are complicated by rapid degradation and clearance of injected pharmacologic agents, so that frequent injections and high concentrations are necessary. Microcarrier-based drug delivery systems, including hydrogels, polymeric microparticles, and liposomes can be used to sustained release of the injected drugs and thereby increase duration and potency of the drug.The liposomes also aid in drug targeting to the microphages and other inflammatory cells, in addition to the above, when the PC headgroups of the injected liposomes in the LD phase they were highly hydratedand therefore are expected to facilitate 2hydrophilic3 lubricationthusimprove joint movements. In our study , we tried to state the benefit of liposomal microcarrier by compareingthe effect of liposomal corticosteroid (Lipotalon ) intra-articular injection with that obtained by corticosteroid injection alone in treatment of internal derangement. Sixteen patients were selected all complaining from ADDWR. After history taking and careful clinical examination, a bite raising splint was constructed for all cases to ensure balanced occlusion and to create room in the posterior part of the joint for the anteriorly displaced disc by positioning the condyle more inferiorly
