Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of General Surgery Summary: Morbid obesity is one of the 21st century epidemics. Surgery is the most important and cost effective treatment. Thromboembolic events such as deep venous thrombosis are a major cause of morbidity and mortality among postoperative patients. Diagnosing (DVT) in severely obese patients is a challenge, as they are difficult to examine physically and often have subtle first signs (silent DVT). Current thromboprophylaxis regimens include elastic stockings, early ambulation, low molecular weight heparin {u2026} etc. The study underhand is to estimate the exact protective value of chemical thromboprophylaxis when added to mechanical thromboprophylaxis to protect against silent DVT post bariatric surgeries, conducted by undergoing bilateral lower limb duplex ultrasonography. Methods: 50 had laparoscopic bariatric surgery ( LSG or MGB ) were prospectively examined for silent DVT incidence by conducting bilateral lower limb venous duplex ultrasonography. In a group of 25 patients, mechanical thromboprophylaxis measures were administered only, while the remaining 25 patients received chemical thromboprophylaxis in the form of subcutaneous low molecular weight heparin injections in addition to the mechanical thromboprophylaxis. Results and conclusion: When using the mechanical thromboprophylaxis only there were 3 established cases of silent DVT detected by duplex in the successive 15 days postoperatively. On the other hand, when using combined mechanical and chemical thromboprophylaxis regimen, it was noted that there was no cases of silent DVT among this group, implementing the high protective value of chemical thromboprophylaxis
