Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Pharmacy - Department of Pharmaceutics Summary: Background: Midodrine is an Ü-agonist prodrug of desglymidodrine used for the management of hypotension. Midodrine has demonstrated usefulness in hepatorenal syndrome. Objective: The objective of this study was to investigate the role of midodrine in patients with non-azotemic cirrhosis with tense ascites and to identify if there was significant improvement in those patients after taking midodrine therapy. The present work also attempted to study clinical pharmacokinetic parameters of midodrine and its active metabolite desglymidodrine in cirrhotic patients with tense ascites which may help in dose selection and improve treatment outcome. Results: significant reduction in body weight and abdominal girth was observed after 2 weeks of midodrine therapy in midodrine group. Statistically significant differences were observed between volunteer group and patient group in AUC₀₋t and Cmax values of midodrine and desglymidodrine. There were no statistically significant differences in T{u00BD} ke , and Tmax between 1/2, e max the 2 groups. Conclusion: Midodrine appeared to be effective in lowering body weights and abdominal girths of non azotemic cirrhotic patients with tense ascites.
