In clinical practice many hypertrophic scars are difficult to manage. And so, this study was performed to evaluate the effect of shock wave therapy on patients with post-burn scars. Subjects: Forty patients were included in this study. Their ages ranged from 20 to 45 years, they were divided into two groups: Group (A) received shockwave therapy and traditional physical therapy, 2 days/week for 6 weeks. Group (B) received traditional physical therapy, 2 days/week for 6 weeks. Assessment: The measurements were done before the study and after 6 weeks of treatment for both groups by using Vancouver scar scale (grading system) and ultrasonography (for scar thickness). Results: The results showed that there was significant reduction in scar thickness with (p value=0.0001) and in total score of Vancouver scar scale with (p value=0.0001). Conclusion: Shock wave therapy is effective in managing immature post-burn scars
