Purpose: to evaluate the efficacy of the conventional versus acupuncture-like TENS on persistent perineal pain post pudendal nerve decompression surgery in males. Methods: Forty five male patients who had persistent perineal pain post pudendal nerve decompression surgery were participated in the study, their ages ranged from 30 to 50 years old, they were randomly divided into 3 equal groups in number, 2 experimental groups (A) and (B) and a control one (C). Group (A) composed of 15 patients received the conventional TENS in addition to the traditional physical therapy and medical care for 3 months. Group (B) received the acupuncture-like TENS in addition to the traditional physical therapy and medical care for 3 months. Group (C) received only the placebo TENS in addition to the traditional physical therapy and medical care for 3 months, TENS was applied once daily, three times per week for 3 months as a total period of treatment, each session was conducted for 10 minutes, two electrodes were positioned over the perineal area between anus and scrotum on the Centrum tendineum, while the other two electrodes were applied under the buttocks medially at the level of the ischial spines (medial to the ischial spines bilaterally) with the patient in comfortable supine hook-lying position with abducted hips. Measurements: serum cortisol level (SCL) and calculation of the Ibuprofen intake (IPI) in mg were measured before and after treatment
