Background: Treatment of acne scars is still a challenge. Autologous Platelet rich plasma (PRP) is a safe and rich medium of growth factors and cytokines which serve in replenishing the lost collagen and elastic fibers. Subcision, also called as subcutaneous incisionless surgery, breaks the fibrotic strands, which tether the scar to the underlying subcutaneous tissue. The depression is lifted by the releasing action of the procedure, as well as from connective tissue that forms during the course of normal wound healing.Objectives: The aim of this study is to evaluate the efficacy of PRP as a monotherapy in treatment of atrophic acne scars and to compare it with the combined use of PRP and subcision in a prospective split-face, clinical study.Patients and methods: 30 patients with bilateral atrophic acne scars were enrolled. Each patient received 3 sessions with one month interval .The right side was treated with PRP alone and the left side received combined therapy with subcision and PRP simultaneously. Evaluation of response was done by comparing patient{u2019}s photos 3 months and 6 months following the last session.Results: Combined treatment with Subcision and PRP showed better results at 6 months, although a non statistical significant difference was detected when comparing the percentage of improvement after 3 and 6 months (p = 0.073). PRP alone showed a statistical significant difference when comparing the percentage of improvement after 3 and 6 months (p =0.046).On comparing both sides a non statistical significant difference was found but better improvement occurred with PRP alone at 3 months and 6 months.Conclusion: PRP monotherapy resulted in better overall scar improvement than combined Subcision and PRP therapy with significant lesser side effects and better patients satisfaction. This effect was evident three months and six months after end of therapy
