Prospective observational study on patients with CLI presenting to the national Institute of diabetes and endocrinology (NIDE). To evaluate the clinical outcome of indirect flow to the foot lesions through indirect revascularization (according to the angiosome principle) following percutaneous transluminal angioplasty of below the knee atherosclerotic lesions. 23 patients (CLI 100%) underwent below knee PTA with indirect revascularization of ischemic tissues according to the angiosome concept with/without a complete pedal plantar arch. The end-point of interest at the end of follow up was limb preservance and amputation free survival as well as the healing progression in the wounds. Successful limb salvage with progressive wound healing was seen in 82.6% in indirectly revascularized limbs. Non-Angiosome based strategies in BTK revascularizations seem to provide acceptable limb preservation rates and encouraging wound healing rates. In addition; whenever possible a patent pedal arch is important to improve the outcome of the IR
