Purpose: To compare the results of femtosecond assisted LASIK and small incision lenticule extraction (SMILE) as regards safety, efficacy and predictability. Design:A prospective, non-randomized, comparative clinical study applied on patients with myopia or myopic astigmatism. Patients and methods:The study included 60 eyes of patients with myopia or myopic astigmatism presenting for refractive error correction. Femtosecond assisted LASIK group included 30 eyes and SMILE group included 30 eyes. The following tests were done for all patients preoperatively and at 3 months postoperatively: full ophthalmological examination, manifest refraction,UDVA, CDVA, corneal topography and corneal higher order aberrations using CSO topography system, corneal biomechanical parameters using ocular response analyzer, assessment for dry eye using schirmer test and tear break up time test (TBUT). Main outcome measures:Uncorrected distance visual acuity (UDVA), distance corrected visual acuity (DCVA), manifest refraction, efficacy, safety and predictability. Results: The mean preoperative spherical equivalent was-4.17(±1.86) in Fs-SMILE group and -3.97(± 2.02) in Fs-LASIK group with no statistically significant difference between both groups (P=0.6914). At 3 months, the mean MRSE in Fs-SMILE group was -0.25±0.38D and the mean MRSE in Fs- LASIK group was -0.183±0.33D and there was no statistically significant difference between both groups (P=0.4975).93% and 93% of eyes in the SMILE and FS-LASIK groups, respectively, were within ±0.5 D; 96.67% and 100% of eyes in the SMILE and FS-LASIK groups, respectively, were within ±1 D
