Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Clinical and Chemical Pathology Summary: Background: worldwide breast cancer comprises 10.4% of all cancer incidences among women, it represents the fifth most common cause of cancer death. Obesity is one of the well-known risk factor for breast cancer development. A recent hypothesis suggests that the adipokines, adiponectin and leptin, are involved in breast cancer development. Various single nucleotide polymorphisms (SNPs) have been identified in the adiponectin gene. Aim of work: We aimed in this study to access the diagnostic value of adiponectin gene polymorphism rs 150 1299 (G267T) in breast cancer and its association to serum adiponectin level in obese and overweight postmenopausal breast cancer female patients. Patients and methods: This study was conducted on 90 patients with breast cancer divided into two groups according BMI. and 60 apparently healthy females as a control group matching in age and in BMI with the cancer group. Both control and breast cancer groups were with BMI >25 ( obese or overweight) . All participant were subjected to full history taking, clinical examination, the histopathological study, laboratory investigations (CA 15-3, serum adiponectin) and molecular study of adiponectin gene rs 1501299 (G276T) by polymerase chain reaction restriction fragment length polymorphism PCR-RFLP technique. Results: A statistically significance difference was observed in the polymorphic genotypes (GT and TT) compared to (GG) wild genotype when compared breast cancer patients to control group with P value 0.001, also On measuring the risk estimate of the polymorphic genotypes (GT and TT) versus wild genotype (GG) we observed a significant difference with P value 0.001
