Background: Focal liver lesions (FLLs) are being discovered at increasing rates due to the wide accessibility of modern high resolution imaging procedures. Elastography is a stiffness imaging modality, which is based on the premise that tissue elasticity can be changed by pathological conditions such as neoplasm or inflammation. Shear wave elastography has been described in the assessment of the mechanical properties of liver tissues as regard liver fibrosis stage but there is limited data on its role in assessing the elasticity of focal liver lesions Objectives: This is Pilot study to evaluate the potential role of point quantification Shear wave elastography (ElastPQ. iU22x MATRIX, Philips) in characterization of hepatic focal lesions and to determine the elasticity values of different focal lesions. Method: The current study was conducted on two hundred participants divided into 4 groups (HCC) group, Hemangioma group, Metastatic group, Others, All patients subjected to full history, clinical examination, laboratory investigations, abdominal ultrasound, assessment of elasticity of the liver focal lesions and the surrounding liver tissue using (ElastPQ. iU22x MATRIX, Philips) Results: ElastPQ.( iU22x MATRIX, Philips) showed that HCC had lower stiffness in comparison to the surrounding liver parenchyma but there is no significant difference in stiffness between the other studied groups and their surrounding liver parenchyma. ElastPQ.( iU22x MATRIX, Philips) cant differentiate between stiffness in different hepatic focal lesions. Conclusion: pSWE (Philips iU22 xMATRIX (Elast PQ) however being a non-invasive, quantitative and non-radiating method for tissue elasticity, it can{u2019}t differentiate between primary and secondary , benign and malignant focal liver lesions
