AFRS is a noninvasive form of fungal rhinosinusitis resulting from an IgE-mediated type 1 hypersensitivity reaction, and a type 3 reaction to fungal antigens in atopic individuals. The classic and still widely accepted diagnostic criteria for AFRS were described by Bent and Kuhn who proposed the following 5 major criteria; nasal polyposis, allergic mucin/eosinophilic mucus with absent tissue invasion of the paranasal tissues, CT findings consistent with chronic rhinosinusitis, positive fungal histologic test or culture results and skin test results suggesting a type 1 hypersensitivity reaction to fungal allergens. Several minor but relevant criteria include a history of asthma, predominance of unilateral disease, radiographic evidence of bone erosion, and peripheral eosinophilia
