Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of General Surgery Summary: Background: Obesity rates have reached epidemic levels with over 300 million obese individuals worldwide. Laparoscopic sleeve gastrectomy (LSG) as a primarily restrictive bariatric surgical procedure has been shown to be effective in producing marked weight loss. However, LSG-associated postoperative gastric leakage and intra and postoperative hemorrhages remain the most important challenges. Staple line reinforcement by inverting sutures has been suggested to reduce these perioperative complications. Our objective was to assess staple line reinforcement by vicryl 2.0 in LSG, focusing on intra and post-operative complications. Methods: Between June 2015 and April 2017, an interventional descriptive case control study was conducted in the Bariatric surgery department of Kasr Alainy hospital, Cairo university. One hundred patients were included in this study. Group A (Control group), fifty patients who had sleeve gastrectomy without reinforcement. Group B (Reinforcement group), fifty patients who had sleeve gastrectomy with reinforcement. Results: The demographic parameters were comparable in the two groups. The overall leak rate was 6% and bleeding rate was 8%. Three cases of gastric leak occurred in Group A and non in Group B. There was three cases of staple line bleeding in Group A that were managed conservatively. The reinforcement operations mean time was 89.96 minute, while it was 57.7 minute in the control group
