Background: In this study, we compare between baska mask versus Air-Q blocker as a conduit for ventilation in adult patients with expected difficult intubation. Methods and material: Thirty four adult patients with expected difficult intubation who are scheduled for elective surgery under general anaesthesia are randomly divided into 2 equal groups: Group A (n=17), in which disposable Air-Q blocker is used for ventilation. Group B (n=17), in which Baska mask is used for ventilation. We recorded: the success rate of each device insertion, the time of insertion of the device, The manipulations required for placement of each device, The oropharyngeal leak pressure, hemodynamic data just before device insertion and after device insertion and appearance of capnography waves to monitor the occurrence of any hemodynamic stress response. Also, we recorded the success rate for insertion of gastric tube through the integrated drainage channel present in both devices and the presence of complications as laryngospasm, breath holding , post-operative blood stains, sore throat or hoarseness of voice is assessed at 0, 2 and 4h. Results: In our study, we found that the Air-Q blocker needed short time for insertion (17.3±4.2 sec) than BASKA mask (19.4±7.1sec). This finding was not statistically significant. Our results regarding success rate of insertion, we found that the success rate of insertion using Air-Q blocker was (100%) which was higher than using baska mask at first trial 89% of insertion
