The problem of nasal airway narrowing at the internal nasal valve is an important issue for patients seeking rhinoplasty. The spreader grafts is a good solution for this problem through maintaining the angle of the internal nasal valve within normal range. We conducted our study in 30 patients seeking rhinoplasty and used spreader grafts in all of them examining their breathing pre and postoperative clinically with minimum follow up period of 8 months. The spreader grafts prevent inferomedial collapse of the upper lateral cartilages after hump excision, maintain the angle of the internal nasal valve within normal range, develop the dorsal aesthetic lines and support the septum after correction of deviation. The spreader grafts are beneficial in patients with high narrow or weak dorsum
