Objective:The aim of this work was to clinically evaluate the efficacy of modified free gingival graft in comparison to subepithelial connective tissue graft in improving the soft tissue around immediately placed dental implants, in the esthetic zone. Materials and methods:Sixteen immediate implants were inserted in sixteen different patients, with the mean age of 36.45, age range (22-54). Sites were divided equally to receive subepithelial connective tissue graft (control) or modified free gingival graft (intervention). Postoperative assessment of the gingival biotype was done using transgingival probing method at nine points in mesial, mid-labial/mid-buccal and distal at 3 different levels (2mm, 4mm and 6mm apical to gingival margin). The keratinized tissue width was measured from the gingival margin till the mucogingival junction,and clinical photographs were taken in order to assess the esthetic outcome three and six months after crown placement using the pink esthetic score method which is based on photographs. Results:The results of this study revealed that there was no statistically significant difference in keratinized tissue width between the two groups at base line, and after 3 months and 6 months after crown placement. However, SCTG group showed statistically significantly lower mean KTW than MFGG group after 1 month as well as 3 months
