This randomized controlled trial was done to evaluate the impact of single versus two-implant mandibular overdenture on the denture retention and success rates of completely edentulous patients. This trial was accomplished following recommendations made in the CONSORT statement for reporting randomized clinical trials. The population of this study consisted of twenty-four patients selected from the outpatient clinic of the Prosthodontic Department Faculty of Oral and Dental Medicine, Cairo University. All patients were completely edentulous patients with ability to provide informed consent; age ranged from 40 to 70 years, free of systemic disease and from any oral pathological lesions in the oral cavity without history of bruxism, clenching, and not taking medical treatment that could affect osseointegration. The patients have been randomly assigned into two groups: - Intervention group: Twelve-patients, each patient received single midline implant mandibular overdentures. - Control group: twelve-patients, each patient received two-implant mandibular overdentures. Implants were placed in canine areas. For both group the opposing maxillary arch restored with conventional complete denture. Digital force meter was used to measure the retention of implant overdentures at baseline, thee, six, and 12 months after loading periods. Digital peri-apical radiograph with Digora software used to measure marginal bone loss at baseline, three, six, and 12 months after loading periods. Plaque and gingival indices were evaluated using plastic periodontal probe at baseline, three, six, and 12 months after loading periods
