Aim: to evaluate endoscopic versus microsurgical resection of third ventricular colloid cysts. Patients and Methods: Retrospective study on 15 patients with colloid cysts of the 3rd ventricle. Results: male-to-female ratio was 1.5:1. Mean age was 33.47 years. 86.7 % had headache and 93.3% had hydrocephalus. Mean cyst diameter was 20 mm.Total resection was 66.7% in endoscopy and 88.9% in microsurgery.Complications rate was 16.7% in endoscopy and 33.3% in microsurgery. No difference in clinical improvement. Mean postoperative hospital stay was 9.5 days in endoscopy and 13.56 days in microsurgery. Conclusion: Endoscopic resection of colloid cysts had lower rate of complications and shorter hospital stay but had lower rate of GTR of the cyst
