Clinical Background: Breast cancer incidence rates are increasing worldwide. The association between high BMI and breast cancer risk is well established in postmenopausal females, but it is less consistent in premenopausal women. In Egypt most of the females above 40 are overweight and obese. The relation between BMI, breast density features, and breast cancer is complex and still need further evaluation. Objectives: This study aimed to explore the relation between BMI, breast density and risk of breast cancer development in an Egyptian Screened Population. Subjects and Methods: A total of 30,443 females attending the Egypt's national breast screening program were included in this study. All females were submitted to demographic and clinical data collection, BMI calculation, mammography screening and breast density assessment (qualitatively) using the American College of Radiology's BI-RADS Classification. Results: BMI in the premenopausal group showed a negative still insignificant statistical relation to breast cancer, while in the postmenopausal group it showed a positive statistically significant relation. For Breast Density, in the premenopausal group there was a positive statistically insignificant difference, while in the postmenopausal group there was a negative highly significant relation. BMI and Breast density were inversely correlated and that was of high significance in both pre and post menopausal groups
