Objectives: The aim of this paper is to compare between the buccal approach versus the palatal approach in augmenting the vertical height of the anterior maxilla. Patients and Methods: Twelve patients were selected and divided randomly into two equal groups; study and control groups. Study group underwent palatal approach while the control group underwent buccal one. Both showed segmental osteotomy for the bone of the anterior maxilla and insertion of alloplastic bone block as grafting material. Four months later the bone gain was measured and compared and implanr insertion was done. The implant primary stability was measured using Osstell. Results: There was no statistically no significant difference between the two groups regarding the percentage of vertical bone height gained after 4 months and there was statistically no significant difference between the two groups regarding implant stability. Conclusion: Segmental sandwich osteotomy technique is an effective technique for augmentation of the anterior maxilla. Within the limitation of our study, the palatal approach has no prevalence over the buccal one regarding the labial prominence or the vertical height.The grafted alveolar ridge provided adequate primary implant stability
