This study was conducted as a double blind randomized clinical trial for the assessment and comparison of post- instrumentation and post-obturation pain following intracanal medications with calcium hydroxide and 2% chlorhexidine gel mixture and calcium hydroxide alone in two visits root canal treatment in mandibular posterior teeth diagnosed with irreversible pulpitis and symptomatic apical periodontitis. Twenty six patients aged between 18-50 years were included in the study. Only mandibular posterior teeth with irreversible pulpitis and symptomatic apical periodontitis were selected. After clinically confirmed diagnosis, patients randomly assigned to the study were treated in two visits: For the first visit, following access cavity preparation and root canal instrumentation using Revo S rotary NiTi instruments, the participants were randomly assigned into two equal groups according to the intracanal medication applied:{u2022}Intervention Group: Calcium hydroxide and 2% chlorhexidine gel mixture.{u2022}Control Group: Calcium hydroxide paste alone. The intracanal medications were applied into the canals with injection syringe system and left inside the canals for one week. Post operative pain was assessed using numerical rating scale (NRS) preoperatively and at 6h, 12h, 24h, 48h, 72h and one week post instrumentation and 6h, 12h and 24h post obturation. Obturation was performed in the second visit using lateral condensation technique. Results revealed that the use of Ca(OH)2 mixed with CHX gel 2% as an intracanal medication caused significant decrease in the degree of post operative pain after (6, 12, 24 and 48) hours post-instrumentation and (6 and 12) hours post-obturation
