The aim of the study : A randomized double blinded clinical trial has been made comparing between 4% articaine with 1:100,000 epinephrine and 2% lidocaine with 1:80,000 epinephrine during treatment of 66 healthy participants suffering from symptomatic irreversible pulpitis of their mandibular molars. 2.Subjects and methods: The participants were randomly allocated to one of two equal groups. Each participant received 2 cartridges of either mepivacaine or articaine anesthetic solution by inferior alveolar nerve block technique.The endodontic procedure started after 15 minutes of injection when the patient reported lip numbness. The participants were asked to record preoperative pain, pain during access cavity preparation and also pain during instrumentation using NRS.The procedure was completed in a single visit. If participant felt moderate or severe pain during access cavity preparation or instrumentation, the procedure was stopped and a supplemental intraligamentary or intrapulpal anesthesia was given
