Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Pediatrics Summary: Background: Kidney transplantation is now considered the treatment of choice for end-stage renal disease (ESRD) in children. New-onset diabetes mellitus (NODM) after transplantation (NODAT) is one of the most negative predictive factors for low survival rate and related co-morbidities. The purpose of this study is to determine the effect of renal transplantation on blood glucose hemostasis. Methodology: This is a prospective study that included 21 renal transplant children and adolescents attending renal transplantation unit at Cairo University Children{u2019}s Hospital. All included cases were subjected to Screening by fasting blood glucose (FBG) and OGTT weekly for4 weeks, then every 3 months for 1 year and annually thereafter HbA1C. Results: The study showed that 14% of these patients developed NODAT. We found significant different between FBG and OGTT in diagnosis of diabetes (p<0.05) and significant association between increased trough level of tacrolimus 6-12 month and impaired OGTT at 12 month post transplantation
