recently the need to minimize surgical trauma, improve patient recovery, reduce the costs without compromising the results of mitral valve repair or replacement represent some of the rationale for minimally invasive procedures. This study primarily aims at determining whether minimally invasive mitral valve surgery through lower mini-sternotomy and mini-thoracotomy improves postoperative outcome when compared to the conventional surgery through full sternotomy. Patients and Methods: 60 patients were included in this study, of which 30 cases were operated upon conventionally through full median sternotomy, 15 cases were operated upon through mini-sternotomy, and 15 cases were operated upon through right anterolateral mini-thoracotomy. Matched groups of patients were used and statistical analysis of the basic data of patients before operations showed no statistical significance between the groups. Early outcomes (during hospital stay) were compared between the groups including intraoperative time parameters, duration of ICU and hospital stay, in-hospital complications (including mortality), drainage, blood transfusion, pain on the 1st and 5th postoperative days, ventilation and inotropic support duration
