Introduction: Patients with type 1 diabetes mellitus (T1DM) have a higher rate of morbidity and mortality compared with the general population, with variations across countries. Diabetic nephropathy (DN) occurs in approximately 30% of diabetic patients and leads to renal failure in most countries. Aim of work is to evaluate urinary osteopontin (OPN) as an early marker for DN in children and adolescents with T1DM. Subjects and methods: This cross-sectional study was carried out on 60 children and adolescent with T1DM > 5years, with age range (7-18 years), they were divided into two groups according to albumin creatinine ratio (ACR), normoalbuminuric: ACR < 30 mg/g (n=30); microalbuminuric: ACR 30-300 mg/g (n=30); together with 30 healthy age and sex matched control. Urinary OPN was measured in patients and controls. Results: The anthropometric and clinical data among the two patient groups showed no significant difference. There was significantly lower weight, height and BMI in patient groups than control groups (P<0.001, <0.001 and 0.005 respectively). Urinary OPN (ng/ml) was significantly higher in microalbuminuric than normoalbuminuric patients (P<0.001). Urinary OPN (ng/mg creat) was significantly higher in patient groups than control groups (P=0.012). There was significant postive correlation between urinary OPN (ng/ml) and ACR (P <0.001)
