Background, a posterolateral thoracotomy is one of the most painful incisions, so patients can suffer considerable pain and complications in the post-operative period if analgesia is not managed appropriately. Our aim was to investigate the role of combined preoperative single dose of Pregabalin and magnesium sulphate in decreasing postoperative pain and morphine consumption in patients undergoing thoracotomy. Methods, One hundred twenty patients with ASA physical status 1 and 2 who underwent thoracotomy incision for cancer lung and pleura were included in this prospective, randomized, and controlled study. Subjects were randomized to four groups, 1st group received oral Pregabalin 300 mg 1 hour before surgery, second group received intravenous infusion of magnesium sulphate 50mg per kg 1 hour before surgery,third group received combination of both drugs and control group. A standard posterolateral thoracotomy incision was used for all patients. Postoperative pain intensity was evaluated by a 0- to 10 horizontal visual analog scale (VAS) (0, no pain; 10, worst imaginable pain). Total morphine consumption in 1st 24 h after operation was also recorded. Results, Visual analog scale scores in the 1st and 4thhour were significantly decreased in combination group compared with the other groups.The 24th total morphine consumption was significantly decreased in combination group compared with the other groups
