Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Orthopaedics Sugery Summary: Tenodesis of the biceps tendon proximal to or within the biceps groove does not address residual synovium in this area, which could act as a persistent pain generator. As a result, arthroscopically assisted open sub-pectoral tenodesis has evolved as a viable method, which allows assessment and treatment of intra-articular pathology, as well as a cosmetically acceptable tenodesis at the distal portion of the inter-tubercular groove. This method also has the advantage of technical simplicity and utilization of an inter-muscular interval for surgical dissection.The sub-pectoral approach permits direct visualization so that the musculotendinous portion of the long head of the biceps can be lined up with the pectoralis major insertion to reproduce optimal resting length. This is critical if strength and a good cosmetic appearance are to be restored. It also fully eliminates the biceps from proximal-based irritation or inflammation.The incision is small, cosmetically pleasing, and hidden in the axillary fold
